经皮肾动脉交感神经射频消融治疗顽固性高血压探讨 被引量:1

摘要 经皮肾动脉交感神经射频消融被认为是安全有效的治疗顽固性高血压的方法。但在目前高血压发病机制尚未完全明确、肾动脉交感神经生理功能尚未完全知晓的情况下,对其进行射频消融治疗,既破坏了血压自身的统一调节,又可致潜在肾动脉损伤风险;且顽固性高血压病变本身不在肾脏,部分顽固性高血压并非药物不可控制,该方法也只是在特定靶点进行降压,循证医学证据不足;故有必要对该方案进行评估,保证患者治疗安全。 Catheter-based renal sympathetic denervation is considered to be a safe and effective method for treating resistant hypertension. But both the pathogenesis of hypertension and the physiological function of renal artery sympathetic nerve are not fully understood, the radiofrequency ablation destroys the self-adjusting of blood pressure and causes the possible risk for renal artery injury; moreover, it is not the disease of kidney that leads to resistant hypertension and part of resistant hypertension can be controlled by drugs; the blood pressure-lowering effect of the method is limited within the specific targets and lacking in sufficient evidence of evidence-based medicine. So it is necessary to re-evaluate the scheme to ensure the safety of patients.
