摘要
目的系统评价外周血中miR-378表达对癌症诊断的潜在价值。方法全面检索PubMed、ISI Web of Science databases、Cochran Library、CNKI和万方等电子数据库,并以文献追溯的方法,收集关于外周血中miR-378的表达与癌症诊断相关的研究,检索时间为各数据库建库起至2014-06-17。采用QUADAS进行文献质量评价,用Meta-Disc1.4及Stata12.0软件对符合条件的所有研究结果进行Meta分析,计算诊断试验相关指标的合并效应量,采用亚组分析探讨异质性的来源,并对纳入文献进行发表偏倚的评估。结果最终纳入7篇文献,共计680例样本。外周血miR-378的合并灵敏度为0.71,95%可信区间(confidence interval,CI)为0.66~0.75,合并特异度为0.68,95%CI为0.62~0.74,合并阳性似然比为2.24,95%CI为1.85~2.71,合并阴性似然比为0.39,95%CI为0.30~0.52,合并诊断比值比为6.33,95%CI为3.75~10.70,合并受试者工作特征曲线下面积为0.77。根据不同检测方法进行亚组分析结果显示,Taqman探针法的灵敏度为0.75,95%CI为0.66~0.83,特异度为0.73,95%CI为0.62~0.82,SYBR Green荧光染料法的灵敏度为0.69,95%CI为0.64~0.74,特异度为0.66,95%CI为0.58~0.73。结论外周血中miR-378的表达对癌症具有较好的诊断价值,其中Taqman探针法优于SYBR Green荧光染料法。
OBJECTIVE To systematic assess the important role of blood-based microRNA-378 (miR-378) in the diagnosis of cancer. METHODS PubMed,ISI Web of Science databases,Cochran Library,CNKI (China National Knowl- edge Infrastructure) and Chinese Wanfang Data were searched to identify eligible studies before June 17,2014. Quality of included trials was assessed by quality assessment of diagnostic accuracy studies (QUADAS). The software Meta-DiSc 1.4 and Stata 12.0 were used to review management and data analysis. Subgroup analysis and publication bias were also e- valuated. RESULTS In total,seven studies were included,with 680 cases involved. Pooled accuracy indicators of sensitivi- ty, specificity,positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio (DOR) with 95 % confidence in- terval (CI) of miR-378 were 0.71(0. 66--0. 75),0. 68 (0. 62--0. 74), 2. 24(1. 85--2. 71),0. 39(0. 30--0. 52), 6. 33 (3.75--10.70), respectively. Area under curve (AUC) of summary receiver operating characteristic (SROC) curve was O. 77. Subgroup analyses indicated Taqman probe assay displayed better diagnostic accuracy than SYBR Green assay [sen- sitivity:0.75(0.66--0.83) vs 0. 69(0.64--0.74),specificity:0.73(0.62--0.82) vs O. 66(0.58--0.73)]. CONCLUTIONS The results of the present Meta-analysis suggested that miR-378 may serve as a novel molecular biomarkers for diagno- sis of cancer; with a relative high level of accuracy, especially based on Taqman probe assay. Further large-scale and multi- centered studies are necessary to validate its potential clinical value for cancer diagnosis.
