摘要
目的探讨颅内蛛网膜囊肿采用神经内镜手术治疗的临床疗效。方法选择本院2010年5月～2014年1月所收治的60例颅内蛛网膜囊肿患者实施回顾性病例对照研究,其中神经内镜手术30例（观察组）,单纯显微外科手术30例（对照组）,观察组直接使用或配合应用神经内镜手术治疗;对照组采用单纯显微外科手术治疗。统计两组患者术后平均住院时间和术后并发症及临床症状改善情况,并随访12个月,观察两组患者术后囊肿复发及变化情况。结果观察组术后平均住院时间为（10.2±1.3）d,明显少于对照组（16.5±2.2）d,P〈0.05;观察组术后发生并发症3例,并发症发生率为10%,明显低于对照组26.7%（8/30）,具有统计学意义,P〈0.05。观察组术后临床症状改善者为25例,改善率为83.3%,与对照组的76.7%（23/30）比较无统计学意义,P〉0.05。观察组术后囊肿复发者1例,复发率为3.3%,明显低于对照组20%（6/30）,具有统计学意义,P〈0.05;观察组术后1年内手术有效者为28例,有效率为93.3%,与对照组的86.7%（26/30）比较无统计学意义,P〉0.05。结论相比单纯显微外科手术治疗来说,直接或配合应用神经内镜手术治疗颅内蛛网膜囊肿可减少并发症及术后囊肿复发,缩短患者术后住院时间。
Objective To discuss the clinical effect of the neuroendoscope operation on treating patients with intracranial arachnoid cyst. Methods A total of 60 patients with intracranial arachnoid cyst who were admitted in our hospital from May 2010 to January 2014 were recruited for the retrospective case control study,with 30 patients receiving the neuroendoscope operation（ observation group） and the other 30 patients going through the pure microsurgery（ control group）. The neuroendoscope was directly applied or in conjunction with others to treat patients in the observation group. The pure microsurgery was operated on patients in the control group. Statistics was carried out on the average hospital stay after the operation as well as postoperative complications and clinical symptoms,improvement of patients in both groups,and there was also a 12 months of follow-up visit to observe postoperative recurrences and changes of cyst in patients of both groups. Results After the operation,the average hospital stay of the observation group lasted for（ 10. 2 ± 1. 3） d,significantly less than that of the control group（ 16. 5 ± 2. 2） d（ P 〈0. 05）. Three patients in the observation group developed postoperative complications and the complication rate was 10%,significantly lower than 26. 7% of the control group（ P 〈0. 05）. A number of 25 patients in the observation group improved in clinical symptoms after the operation and the improvement rate was 83. 3%,thus there was no statistical significance when compared with 76. 7% of the control group（ P 〉0. 05）. One patient in the observation group had cyst once again after the operation and the recurrence rate was 3. 3%,greatly lower than 20% of the control group（ P 〈0. 05）. Within one year after the operation,28 patients in the observation group had responses to the therapy and the efficient rate was 93. 3 %,thus there was no statistical significance when compared to 86. 7 % of the control group（ P 〉0. 05）. Conclusion Compared to pure microsurgery,direct or combined application of neuroendoscope to treating patients with intracranial arachnoid cyst can reduce complication and cyst recurrence,as well as patients＇ postoperative hospital stay.
