摘要
目的 应用实时三维超声心动图(RT-3DE)测量房间隔缺损(ASD)的缺损大小并评价介入封堵术心室功能的变化。方法 对19例行介入封堵治疗的房间隔缺损患者行RT-3DE检查,测量房间隔缺损的大小;记录术前1 d、术后2~3 d、术后1周、术后1个月、术后3个月的左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室每搏输出量(LVSV)、左室射血分数(LVEF)、右室舒张末期容积(RVEDV)、右室收缩末期容积(RVESV)、右室每搏输出量(RVSV)、右室射血分数(RVEF)变化。结果 LVEDV、LVESV、LVSV从术后2~3 d至术后3个月逐渐增加;RVEDV、RVESV以及RVSV从术后2~3 d起逐渐减少。术后1周、术后1个月、术后3个月的LVEDV、LVESV、LVSV、RVEDV、RVESV、RVSV与术前比较,差异有统计学意义(P〈0.05),LVEF和RVEF在手术前后变化不明显(P〉0.05)。RT-3DE测量缺损最大径结果 与手术测量值相关性好(r=0.9903)。结论 RT-3DE对准确测量缺损最大径并有效评估ASD患者介入术前后左右心室功能变化,具有重要应用价值。
Objective To observe the application of real time three-dimensional echocardiography(RT-3DE) technology inventricular systolic function assessment and defect size measurement in patients with atrial septal defect(ASD). Meth- ods 19 patients with ASD were detected by RT-3DE at 1 day before the occlusion,2 -3 day,1 week,1 month,3 month after the occlusion to measure the defect size and ventricular systolic function. Results Left ventricular end-diastolic vol- ume (LVEDV) ,left ventricular end-systolic volume ( LVESV ), left ventricular stroke volume ( LVSV ), right ventricular end-diastolic volume (RVEDV), right ventricular end-systolic volume (RVESV) and right ventricular stroke volume (RVSV) changed gradually from 2 - 3 day after the treatment. The values of LVEDV, LVESV and LVSV at 1 week, 1 month,3 month after were significantly higher compared with before (P 〈 0.05 ). RVEDV, RVESV and RVSV from 1 week to 3 month after the occlusion significantly increased compared with before ( P 〈 0.05 ). Left ventricula rejection fraction (LVEF) and right ventricula rejection fraction (RVEF) had no significant difference (P 〉 0.05 ). The defect diameters measured by RT-3DE were correlated well with surgery result ( r = 0. 9903 ). Conclusion RT-3 DE can measure defect size accurately and assess the ventricular systolic function effectively.
