摘要
目的:观察手术结合中药治疗精索静脉曲张合并精浆中性α-糖苷酶异常的不育症患者的临床疗效。方法:将60例符合纳入标准的精索静脉曲张并伴精浆中性α-糖苷酶异常的不育患者随机分为手术组(对照组A,n=20)、聚精汤组(对照组B,n=20)、手术与聚精汤结合组(治疗组,n=20)。手术均采用改良Palomo术,中药均以聚精汤(徐福松方)口服,一日一剂,分2次服用。三组患者共治疗12周,治疗结束后随访48周,观察其精液体积、精液p H值、精子总数、精子浓度、精子前向活动率(PR)、总活动率(PR+NP)、精浆中性α-糖苷酶活性及配偶怀孕情况等。结果:三组在治疗后组间对照比较,组间患者的精子浓度、精子前向活动率(PR)、总活动率(PR+NP)、精浆中性α-糖苷酶活性(一次射精)均有显著性的差异(P<0.05),并且都以手术与聚精汤结合组为优。而精液体积、精液p H值无显著性差异(P>0.05)。三组治疗后的精子浓度、精子前向活动率(PR)、总活动率(PR+NP)、精浆中性α-糖苷酶活性(一次射精)与治疗前相比均有明显提高,有显著性差异(P<0.05)。精液量、精液p H值与治疗前相比无显著性差异(P>0.05)。60例患者随访48周,其配偶怀孕共21人(总怀孕率35%),其中手术组6人(怀孕率30%);聚精汤组2人(怀孕率10%);手术与聚精汤结合组13人(怀孕率65%);三组间比较有显著性差异(P<0.01),手术与聚精汤结合组优于聚精汤组或手术组。三组的总体疗效差异性显著(P<0.01或P<0.05),手术与聚精汤结合组的疗效最优。结论:手术结合聚精汤能够提高精索静脉曲张不育合并精浆中性α-糖苷酶异常患者的精液质量和精浆中性α-糖苷酶活性,改善睾丸附睾功能提高精子活力,同时也能提高配偶怀孕率;效果优于单纯手术或单纯中药治疗。
Objective： Observe Surgery combined with Chinese traditional medicine treatment for varicocele with abnormal Seminal plasma neutral alpha-glucosidase infertility in patients with clinical efficacy. Methods： Random divide 60 Varicocele infertility patients with the an abnormally level Seminal plasma neutral a- glucosidase into surgery group （control groupA, n=20）, JuJing Tang group （control groupB, n=20）, Surgery combined with JuJing Tang group （n=20）, all the surgery used improved Palomo surgery. They all take orally the traditional Chinaese medicine JuJing Tang. Three groups of patients were treated for 12 weeks, follow-up after 48 weeks. Semen volume, Semen pH, total sperm number and sperm concentration and Forward motion（PR）, Total motility （PR＋NP）, survival rate,Normal morphology sperm percentage, Seminal plasma neutral a-glucosidase and pregnancy are observed objects. Results： Compared with the three control groups, there are significant difference of sperm concentration among different groups （P〈0.05） Forward motion （PR）, Total motility （PR＋NP）, Seminal plasma neutral a-glucosidase （one time ejaculation） expressed significant differences （P〈0.05）, the group of combined treatment of surgery and JuJing Tang is better. There is no significant differences of Semen volume, Semen pH was no significant difference （P〉0.05）. There is significant differences Sperm concentration, Forward motion （PR）, Total motility （PR＋NP）, Seminal plasma neutral a-glucosidase （one time ejaculation） （P〈0.05）. There is no significant differences of Semen volume, Semen pH （P〉0.05）. After the Follow-up visits 48 weeks later, 21 ＇s spouses are pregnant （Pregnancy rates 35%）, among of them, 6 are from the surgery gooup 30%, JuJing Tang group 2 peoples （Pregnancy rates 10%）, Surgery combined with JuJing Tang group 13 peoples （Pregnancy rates 65%）. There are significant difference among the three groups（P〈0.01）. The effect of the group of treatment group is better than the other two groups. The overall effect of the three groups is significant different. （P〈0.01 or P〈0.05）, among all, the curative effect of combination of surgery and JuJing Tang is the best. Conclusion： Surgery combined with Jujing Tang can improve infertility with varicocele merge Seminal plasma neutral a-glucosidase of patients with abnormal semen quality and Seminal plasma neutral a-glucosidase activity, improve the function of testis and epididymis improve sperm motility, can also improve your spouse pregnancy rates; the effect is better than surgery or simple Chinese medicine treatment.
