摘要
目的比较两种不同荷包缝合技术在吻合器痔上黏膜环切术（procedure for prolapse and hemorrhoids,PPH）治疗重度痔中的临床效果。方法回顾性分析2011年9月-2014年9月采用PPH治疗的140例重度混合痔患者的临床资料,均为Ⅲ-Ⅳ期脱垂型内痔,根据术中采取的不同荷包缝合技术分为两组：观察组70例,PPH术中采用双荷包缝合技术;对照组70例,采用单荷包缝合技术。比较两种不同荷包缝合技术的临床疗效。结果观察组采用双荷包缝合后切除痔上直肠黏膜的宽度、均匀程度、痔核回缩率均明显优于对照组（P〈0.05）,而术中出血、住院时间、吻合口狭窄和总体疗效比较,两组均无统计学差异（P〉0.05）;观察组平均手术时间明显长于对照组（P〈0.05）。结论 PPH手术治疗重度痔,术中采用双荷包缝合技术虽然手术时间有所延长,但切除痔上直肠黏膜的宽度、均匀程度和痔核回缩率明显优于单荷包缝合。
Objective To investigate the role of two different purse-string suture techniques during the procedure for prolapse and hemorrhoids（ PPH） in the treatment of severe prolapse hemorrhoids. Methods The clinical and follow-up data of 140 severe mixed hemorrhoids patients with grade Ⅲ to Ⅳ prolapsed internal hemorrhoids treated by PPH were analyzed retrospectively from September 2011 to September 2014. According to different purse-string suture techniques,they were divided into two groups with 70 cases for each. The observation group were performed double purse-string suture and the control group were performed single purse-string suture during PPH. The clinical outcomes were compared between two group. Results The width and equitability of excisional rectal superior hemorrhoidal mucous membrane and retraction rate of hemorrhoids were superior in observation group than those in control group（ P〈0. 05）,the operative duration of observation group was longer than that of control group（ P〈0. 05）. While there was no statistical difference in intra-operative blood loss,hospital stays,anastomotic stoma stenosis rate and total curative effect between two groups（ P〉0. 05）.Conclusion In spite of longer operating time,the technique of double purse-string suture is significantly superior than single purse-string suture during PPH for severe hemorrhoids,in the aspects of the width and equitability of excisional rectal superior hemorrhoidal mucous membrane and retraction rate of hemorrhoids.
