摘要
目的：观察梗阻性黄疸（obstructive jaundice,OJ）患者术后早期肠内营养对其血清细胞因子的影响。方法：入选2010年5月~2012年5月我院手术治疗的梗阻性黄疸20例,术后予早期肠内营养（EN组）,另20例梗阻性黄疸术后予肠外营养（PN组）作对照,检测发病2d、第7天、第9天、第14天血清白介素2（IL-2）、白介素4（IL-4）、白介素6（IL-6）、白介素8（IL-8）、白介素10（IL-10）、干扰素γ（INF-γ）、肿瘤坏死因子-α（TNF-α）的水平。结果 ：PN组患者病程第7天血清IL-6、IL-8、TNF-α[（7.0±3.7）pg/mL、（55.1±38.5）pg/mL、（42.3±28.2）ng/mL）]高于EN组[（3.3±1.5）pg/mL、（33.4±16.4）pg/mL、（33.5±21.0）ng/mL,P〈0.05）];病程第9天血清IL-6、IL-8、TNF-α[（11.8±3.5）pg/mL、（56.9±37.6）pg/mL、（40.3±30.1）ng/mL]也高于EN组[（4.1±1.3）pg/mL、（25.5±22.3）pg/mL、（27.1±18.6）ng/mL,P〈0.05）]。结论：梗阻性黄疸术后早期肠内营养可减少术后血清炎性细胞因子水平,是减少术后并发症,促进患者早期康复的可能机制。
Objective： To investigate the effects of early enteral nutrition on the serum cytokine levels of patients after operations for obstructive jaundice. Methods： The total of 40 patients with obstructive jaundice were enrolled from May 2010 to May 2012 and divided into 2 groups averagely. They were treated with early enteral nutrition （EN） or parenteral nutrition （PN）. IL-2, IL-4,IL-6,IL-8,IL-10,INF-γ and TNF-a levels in serum were tested in all patients. Results： The levels of IL-6, IL-8 and TNF-α of patients with PN [（6.9±3.5） pg/mL, （55.1±38.5） pg/mL and （41.3±30.1） ng/mL] were higher than that of patients with EN[（4. 0±1. 3） pg/ml, （33.2±16.2） pg/mL and （32.5±21.1） ng/mL, P〈0.05] on the 7th day. The levels of IL-6,IL-8 and TNF-α of patients with PN E（11.5±3.2） pg/mL, （55.1±38.5） pg/mL, （41.3±30.1） ng/mL] were also higher than that of patients with EN [（3.2±1.5） pg/mL, （23.5±24.3） pg/mL, （28.3±22.6） ng/mL, P〈0.05, P〈 0.05] at the 9th day . Conclusions： The low and persistent serum IL-6,IL-8 and TNF-α levels may be attribute to the recovery of patients after operations for obstructive jaundice. Key words：
