糖尿病大鼠禁食性尿白蛋白对评价早期糖尿病肾病的意义 被引量:1

Experimental study in diabetic rats of fasting urinary protein to evaluate the early nephropathy

摘要 目的探讨糖尿病大鼠禁食性尿蛋白对评价早期糖尿病肾病的意义。方法成年雄性Sprague-Dawley大鼠57只,随机分为早期糖尿病肾病组(n=33)和对照组(n=24)。早期糖尿病肾病组大鼠一次性腹腔注射链脲佐菌素建立模型。2组大鼠分别于第4周、8周、12周、16周禁食24 h,留取禁食状态下24 h尿液。放免法检测尿白蛋白,计算24 h尿蛋白、尿白蛋白排泄率和尿白蛋白/肌酐比值,同时检测肾脏内生肌酐清除率、肾肥大指数、肾小球细胞外基质堆积指数。电镜观察肾小球基底膜厚度。结果早期糖尿病肾病大鼠不同时间点的24 h尿蛋白、尿白蛋白排泄率和尿白蛋白/肌酐比值均显著高于对照组大鼠(P<0.05),组间差异与时间变化无交互作用(P>0.05);这3项指标均与内生肌酐清除率、肾脏肥大指数呈正相关(P<0.05)。结论禁食性24 h尿蛋白、尿白蛋白排泄率和尿白蛋白/肌酐比值均是评价早期糖尿病大鼠肾病有价值的指标。 Objective To study the significance of fasting urinary protein to evaluate the early nephropathy in diabetic rats.Methods Fifty-seven male adult Sprague-Dawley rats were randomly divided into early diabetic nephropathy group (n =33) and control group (n =24).Early diabetic nephropathy rats were produced byone-time intraperitoneal injection of streptozotocin and were confirmed.At the end of 4 w,8 w,12 w a nd 16 w,rats were fasted for 24 hours,and total 24 hour urine was collected.Urine albumin were measured by immunoturbidimetry,and the urinary total protein,urinary albumin excretion rate and urinary albumin / creatinine ratio were calculated accordingly.The corresponding creatinine clearance,renal hypertrophy index,glomerular extracellular matrix accumulation index were monitored.The histology changes were examined with light microscopy,and the glomerular basement membrane thickness was observed by electron microscopy.Results The fasting levels of urinary total protein,urinary albumin excretion rate and urinary albumin to creatinine ratio in early diabetic nephropathy rats were significantly higher than those of normal rats in each time point of early diabetic nephropathy development (P < 0.05).The differences between two groups showed no interaction with time points (P > 0.05).Furthermore,the levels of fasting urinary total protein,urinary albumin excretion rate and urinary albumin to creatinine ratio in early diabetic nephropathy rats were positively correlated with glomerular filter rate and renal hypertrophy index (P < 0.05).Conclusion These data indicate that fasting urinary total protein,urinary albumin excretion rate and urinary albumin to creatinine ratio in diabetic rats can be used as a valuable indicator to evaluate early diabetic nephropathy.
