重症肺炎合并低氧血症是急性呼吸窘迫综合征吗 被引量：8

摘要 在呼吸与危重症的学术界一直有这样的争议：合并低氧血症的重症肺炎是不是ARDS？支持者认为合并低氧血症的重症肺炎应该就是肺源性ARDS,而且是肺源性ARDS的最主要病因;而反对者则认为,重症肺炎尤其是细菌性肺炎,虽有多肺叶病变但肺泡结构常没有明显损伤,即使部分重症肺炎因局灶性肺炎继发的炎性瀑布反应导致其他肺叶或肺段的损伤,继发病变应该属于肺外源性ARDS的特点.因而国外许多权威文献在描述急性低氧性呼吸衰竭时,往往把重症肺炎和ARDS分开来写,说明二者确有不同之处.因此,我们需要认真思考并探究二者的异同.
