军事训练相关性膝关节隐匿骨折的MRI评价 被引量:3

摘要 目的:探讨军事训练致膝关节隐匿骨折的MRI价值及临床意义。方法回顾分析X线检查阴性的150例军事训练伤患者行1.5TMR膝关节检查。患者均为男性,年龄18~32岁,平均21.5岁。 MRI检查时间自创伤后3~35d,平均14d。结果所有病例MRI关节周围均见异常信号。56例表现为隐匿骨折,其中32例伴有关节腔积液,27例伴半月板撕裂,22例伴前交叉韧带损伤,8例伴侧副韧带损伤。37例随访时间1~18个月,23例局部骨质异常信号消失,14例局部异常信号减弱。结论 MRI能早期、准确显示军事训练致膝关节隐匿骨折及其伴发损伤,并可以反映隐匿骨折的病理改变,对临床诊断治疗及指导其康复训练具有重要价值。 Objective To investigate the value and clinical significance of MRI in occult fracture of knee joint caused by military training .Methods Totally 150 patients,who failed to reveal suspected fractures from mili-tary training injury with X-ray plain films and underwent 1.5T MR knee examination ,were retrospectively analyzed . All patients were male with age ranged from 18 to 32 years,averaging 21.5 years.MRI examination was conducted 3 days to 35 days after injury ,with an average of 14 days.Results All cases had positive radiographic founding a-round the knee joints in MRI .Fifty-six cases were occult fracture , among them 32 patients with joint cavity effu-sion,27 cases with meniscus tear ,22 cases with anterior cruciate ligament injury and 8 cases with collateral ligament injury.Thirty-seven patients were followed up for 1 to 18 months,among whom 23 cases of local bone abnormal sig-nal disappeared ,14 cases of local abnormal signal weakened .Conclusion MRI can accurately display occult frac-ture of knee joint and its concomitant injury caused by military training in the early stage ,and can reflect the patho-logical changes of occult fracture .It has great value in the clinical diagnosis ,treatment and rehabilitation training .
