摘要 目的:探讨Wagner翻修股骨柄在髋关节股骨侧翻修中的临床应用效果。方法:2009年9月至2013年1月共对10例患者的10个髋关节进行了翻修手术。男6例,女4例。年龄45~76岁,平均61.2岁。感染后翻修2例,臼杯及柄无菌性松动6例,假体周围骨折2例。采用改良Harris评分评价髋关节功能,X线评价术后假体有无松动、下沉和骨愈合情况。股骨侧假体选择Wagner翻修股骨柄。结果:术后切口均Ⅰ期愈合。本组10例患者均获得随访,随访时间6~36月,平均19.2月。植骨处愈合,无明显骨吸收及假体下沉。无感染、脱位、深静脉血栓形成等并发症。Harris评分由术前的45.7±10.77分提高到术后87.10±4.68分,有统计学意义。结论:Wagner在人工髋关节股骨侧翻修术中,可获得良好的远端固定,近期疗效满意。 Objective: To discuss the clinical effect of Revision Hip Stem(Wagner SL) in revision arthroplasty of the femoral component. Methods: From Sep 2009 to Jan 2013, 10 cases were treated with acetabular revision, including 6 males and 4 females with an average age of 61.2 ranging from 45 to 76 years.There were 2 cases of infection after hip arthroplasty, 6 cases of the loosening of the cup and stem, 2 cases of periprosthetic fracture. Harris scoring system was adopted to evaluate the function of hip joints. Radiograph was adopted to evaluate loosening,subsidence and bone healing of the prosthesis. Revision Hip Stem(Wagner SL) was used. Results: All wounds were healed by first intention.10 patients were all followed-up, from 6 months to 36 months, with an averaged of 19.2 months, grafted bones healed, without loosening or subsidence. No complications such as infection,dislocation or deep venous thrombosis happened. Harris hip score increased from preoprative 45.7 ±10.77 points to postoprative 87.10 ±4.68 points, showing significantly difference(P 0.05).Conclusion: The Revision Hip Stem(Wagner SL) in revision could achieve good clinical distal fixation arthroplasty of the femoral component. The short-term effects were remarkable.
