摘要
目的了解住院拟化疗消化道肿瘤患者的营养状况和影响因素及其与生活质量的关系,为进一步制定营养干预方案提供依据,以提高患者化疗期间的生活质量。方法便利选取2011年12月至2013年6月住院拟行化疗的消化道肿瘤患者114例,采用主观全面营养状态评价量表(subjective global assessment,SGA)和癌症患者生活质量评价量表第3版(quality of life-core 30questionnaire version 3.0,EORTC QLQ-C30V3.0)中文版进行调查。结果经SGA评定为营养不良者有79例(占69.2%),学历(初中及以下)、TNM分期(Ⅲ~Ⅳ)、卡氏体能状况(Karnofsky performance status,KPS)评分(<80分)为营养不良的影响因素;营养不良组的总体状况、躯体功能、角色功能、情绪功能及社会功能得分低于营养良好组,疲乏、恶心呕吐、疼痛、失眠、食欲下降、便秘、腹泻及经济困难得分高于营养良好组,差异均有统计学意义(P<0.05)。结论部分住院拟化疗的消化道肿瘤患者存在营养不良,从而导致生活质量的下降,应针对营养状况的影响因素给予相应的营养干预,以改善患者的营养状况,达到提高其生活质量的目的。
Objective To describe the nutritional status and explore influencing factors of nutritional status and investigate its effect on quality of life for inpatients with alimentary tumor before chemotherapy so as to provide reference for targeted interventions and improve their quality of life.Methods By convenience sampling,114 inpatients were selected and investigated by SGA and EORTC QLQ-C30(version3.0).Results The incidence of malnutrition among the 1 1 4 inpatients was 6 9 .2%(7 9/1 1 4 ).Educationmiddle school or under),TNM stage(III^IV),KPS score(〈80)were the influencing factors of malnutrition.People with malnutrition status received lower scores than the adequate nutritional status in general health status, physical functioning,role functioning,emotional functioning,and social functioning.The fatigue,nausea, vomiting,pain,insomnia,loss of appetite,constipation,diarrhea and financial difficulties scores were higher than adequate nutritional group,and the differences were statistical significance(P〈0.05).Conclusion Part of the alimentary tumor inpatients before chemotherapy are of malnutrition,and malnutrition can lead to decreased quality of life.Appropriate nutritional interventions should be given to reduce the adverse factors and to improve the inpatients’nutritional status and improve their quality of life.
