比较种植支抗和摇椅弓在矫治成人安氏Ⅱ2错(牙合)中压低上前牙的临床效果 被引量:7

Comparision of intrusion effect on the maxillary incisors between Rocking Chair Archwire and Miniscrew Anchorage for Adult Angle Class Ⅱ Division 2 Malocclusion

摘要 目的:观察种植支抗和摇椅弓在矫治成人安氏Ⅱ2错牙合中压低上前牙的临床效果。方法:18例安氏Ⅱ类2分类错牙合患者,随机分为两组,一组使用种植支抗压低上前牙,另一组使用摇椅弓压低上前牙。头影测量描记上切牙及上颌第一磨牙的位置变化。结果:在打开前牙咬合,压低上颌前牙阶段种植支抗组平均时间7.2周,摇椅弓组平均时间12.8周,种植支抗组的疗程短于摇椅弓组。两组病例治疗前后覆盖,覆牙合,U1-PP,U1-SN,U1-Stms有显著性变化。相比摇椅弓组,种植支抗组测量指标变化更为明显的是U1-PP(P<0.01)和U1-Stms(P<0.01),覆盖(P<0.05)和U1-SN(P<0.05)也显著性减少。而U6-PP,U6-SN差异无统计学意义。结论:种植支抗与摇椅弓在矫治成人安氏Ⅱ2错牙合中压低上前牙均有明显的效果,但种植支抗组在压低上前牙、改善唇齿关系及缩短疗程方面优于摇椅弓组。 Objective:To compare the intrusion effect on the maxillary incisors,rocking chair archwire and miniscrew anchorage were used to correct the adult angle Class Ⅱdivision 2 malocclusion. Method:18 adult patients with Class Ⅱdi-vision 2 malocclusion were chosen and divided into two groups randomly:miniscrew group and rocking chair archwire group. 9 patients in the miniscrew group and 9 patients in the rocking chair archwire group were analyzed for maxillary incisors in-trusion. The upper 1st molars displacement and the relative skeletal changes were analyzed in the horizontal and vertical di-rections in both groups. Result:In average,the treatment period was 7.2 weeks in miniscrew group and rocking chair arch-wire group was 12.8 weeks.Significant changes of overjet,overbite,U1-PP,U1-SN,U1-Stms were observed in two groups before and after treatment.However,there were significant greater changes in U1-PP (P &lt;0.01),U1-Stms (P &lt;0.01) in the miniscrew group than in the rocking chair archwire group and there were significant reductions in overjet (P&lt;0.05),U1-SN (P &lt;0.05).There was no significant difference in U6-PP and U6-SN. Conclusion:There were significant intrusion effect on the maxillary incisors by using miniscrew anchorage and rocking chair archwire in treatment adult angle Class Ⅱdivision 2 malocclusion. However,in intruding the upper incisors,improving the lip-teeth relationship and shorting the course of treat-ment ,miniscrew anchorage was better than rocking chair archwire.
