3Dmax补片在腹腔镜下全腹膜外腹股沟疝修补术中不同固定方法的临床疗效 被引量:11

Clinical effect of 3Dmax patch in laparoscopic full extraperitoneal inguinal hernia repair with different fixation methods

摘要 目的探究3Dmax补片在全腹膜外疝修补术(TEP)中不同固定方法的临床疗效。方法选取武汉市第五医院2011年9月~2013年9月收治的150例腹股沟疝行TEP术患者,根据3Dmax补片固定方法不同分为钉合器固定组、Prolene缝线固定组、生物胶固定组,各50例,观察比较各组临床疗效。结果生物胶固定组与Prolene缝线固组、钉合器固定组手术时间分别为(1.52±0.11)、(2.01±0.22)、(2.92±0.42)h,术中出血量分别为(12.5±7.2)、(24.5±8.6)、(34.2±12.2)mL,患者住院总费用分别(625.8±75.2)、(7125.2±98.2)、(8253.6±125.3)元,术后第1天疼痛评分分别为(1.1±0.5)、(1.8±0.9)、(2.5±1.1)分。与钉合器固定组比较,生物胶固定组及Prolene缝线固定组手术时间更短,术中出血量更少,住院费用更低,术后第1天疼痛评分更低,差异均有统计学意义(均P〈0.05);且生物胶固定组较Prolene缝线固定组效果更好(均P〈0.05)。三组术后住院时间、拔除尿管时间、患者满意度及术后并发症比较,差异均无统计学意义(P〉0.05)。结论腹腔镜腹股沟疝修补术中3Dmax补片钉合器固定、Prolene缝线固定与生物胶固定的对比研究中,生物胶固定模式为最优选择方法。 Objective To study the clinical effects of 3Dmax patch in totally extraperitoneal prosthesis (TEP) of differ-ent fixation methods. Methods 150 patients with inguinal hernia taking TEP admitted to the Fifth Hospital of Wuhan City from September 2011 to September 2013 were chosen and divided into stapling devices group, Prolene suture fixa-tion group, biological glue fixation group according to different fixation methods of 3Dmax patch, with 50 cases in each group. The clinical effects of all groups were observed and compared. Results In the biological glue fixation group, Pro-lene suture fixation group, stapling devices group, the operative time were (1.52±0.11), (2.01±0.22), (2.92±0.42) h re-spectively, intraoperative blood loss were (12.5±7.2), (24.5±8.6), (34.2±12.2) mL respectively, the hospitalization total cost were (625.8±75.2), (7125.2±98.2), (8253.6±125.3) yuan respectively, one day postoperative pain scores were (1.1±0.5), (1.8±0.9), (2.5±1.1) points respectively. Compared with stapling devices group, the biological glue fixation group and Prolene suture fixation group had shorter operation time, less intraoperative blood loss, lower hospitalization total cost, lower one day postoperative pain scores, the differences were all statistically significant (P〈 0.05), and the biological glue fixation group was much better than Prolene suture fixation group (P〈0.05);there were no significant differences in the postoperative hospitalization days, time of catheter removal, the satisfaction of patients and incidence of compli-cations among the three groups (P〉0.05). Conclusion Among comparative study of laparoscopic inguinal hernia repair 3Dmax patch with stapling devices, Prolene suture fixation and biological glue fixation, the mode of biologi-cal glue fixation is the optimal selection method.
