光学相干断层扫描仪在老年白内障中的临床应用价值 被引量:2

Clinical value of optical coherence tomography on cataract in elderly patients

摘要 目的分析光学相干断层扫描仪(OCT)在老年白内障患者术前前房深度(ACD)与晶状体厚度(LT)测量中以及术后黄斑区结构扫描中的的应用价值。方法选择我院收治的80例(92眼)白内障患者为研究对象,于患者术前采用OCT对患者ACD与LT进行测量,采用变异系数(CV)及类内相关系数(ICC)评价OCT测量的一致性和可重复性。同时,于术后采用OCT对患者黄斑中心凹视网膜形态进行扫描观察,客观评价患者术后的黄斑病变情况及视力情况。结果白内障患者术前ACD与LT行OCT测量的一致性和可重复性较好。同时,术后黄斑区域OCT扫描结果显示,60眼(65.22%)黄斑区未见异常;32眼(34.78%)发生黄斑区病变。术后,60眼黄斑区未见异常患者的视力较术前明显提高(P<0.05),而32眼黄斑区发生病变患者的视力较术前下降(P<0.05)。结论 OCT在白内障患者术前ACD与LT测量中具有较好的一致性和可重复性,精确可靠,且可为白内障术后黄斑病的早期诊断及术后视力预判提供可靠依据。 Objective To analyze the clinical value of optical coherence topographer (OCT) in preoperative anterior chamber depth and the lens thickness measurement, and in the application of postoperative maeular area structure scanning in elderly patients with cataract. Methods Eighty cases (92 eyes) of cataract in our hospital were selected, and anterior chamber depth and the lens thickness were measured by OCT before surgery. The coefficient of variation (CV) and correla- tion coefficient (ICC) were used to assess the consistency and repeatability of the measurement. At the same time, macular center concave structure was scanned to evaluate the postoperative visual acuity and macular disease after surgery. Results ACD and LT showed that consistency and repeatability of OCT were good. Postoperative macular area OCT scanning showed 60 eyes (65. 22%) in the macular area were normal, and 32 eyes (34.78%) occurred disease in the macular area. Postoperatively, visual acuity of 60 normal eyes was improved obviously (P〈0. 05), and visual acuity of other 32 eyes decreased (P〈0. 05). Conclusions OCT has good consistency and repeatability in the anterior chamber depth and the lens thickness measurement, and can provide reliable evidence for the early diagnosis of macular disease and postoperative vision.
