超声心动图VFM技术在评估扩张性心肌病患者左室血液流场变化中的应用 被引量:2

Application of echocardiography VFM technology for left ventricular blood flow changes in patients with dilated cardiomyopathy

摘要 目的:应用血流向量成像(VFM)技术检测扩张型心肌病患者和正常人左室腔内血液流场变化。方法:收集扩张型心肌病患者40例为试验组,同时收集同期健康志愿者40例为对照组。首先采用常规心动图检测,收集左室腔内血流向量图,应用VFM技术测量涡流横径、纵径和圈数,并测量左室流出道基底段、中间段、心尖段向量的大小,比较两组差异。结果:试验组涡流圈数较对照组明显增多[(11.32±5.86)∶(3.65±1.85),P<0.05]。试验组涡流纵径和横径均明显高于对照组[纵径:(45.42±22.01)mm∶(16.78±8.42)mm;横径:(39.16±14.52)mm∶(9.34±4.12)mm;均P<0.05]。试验组基底段向量速度显著低于对照组[(5.23±0.98)cm/s∶(21.13±10.01)cm/s,P<0.05],而中间段明显高于对照组[(16.19±8.52)cm/s∶(12.23±7.74)cm/s,P<0.05],两组心尖段向量速度差异无统计学意义[(1.48±0.51)cm/s∶(1.61±0.42)cm/s]。结论:VFM技术能够直观检测左室腔内血液流场变化,同时可测量局部向量速度,为定量评价扩张型心肌病患者左室腔内血液流场变化提供依据。 Objective: To detect the blood flow changes of left ventricular cavity in patients with dilated cardiomyopathy and healthy volunteers with flow vector imaging (VFM). Method: Experimental group included 40 patients with dilated cardiomyopathy, and control group included 40 healthy volunteers. Conventional echocardio- graphy was used first, then vector diagram of left ventricular cavity blood was collected. The technology of VFM was used to measure vortex diameter, longitudinal diameter and number of turns, and the basal segment, the mid- dle segment, apical segment vector magnitude of left ventricular outflow tract difference were measured and com- pared between two groups. Result: Eddy current lap of experimental group increased significantly compared with control group [(11.32±5.86)vs(3.65±1.85),P〈0.05]. Eddy current longitudinal diameter and transverse di- ameter were significantly higher than those in control group [ longitudinal diameter: (45.42 ± 22.01)mm vs(16.78 ±8.42)mm;transverse diameter: (39.16±14.52)mm vs(9.34±4.12)mm;both P〈0.05]. Speed vector of basal segment in experimental group was significantly lower than that in control group [(5.23±0.98)cm/s vs(21.13± 10. 01)cm/s,P〈0. 05], while the middle section was significantly higher than that in control group [(16.19± 8.52)cm/s vs (12.23±7.74)cm/s,P〈0.05] . The apical segments of vector velocity of the two groups were not statistically different [(1.48±0.51)cm/s vs (1.61±0.42)cm/s]. Conclusion: VFM technology can visually detect blood flow changes in the left ventricular cavity and measure local velocity vector. It provides the reference for quantitative evaluation on blood flow changes of left ventricutar cavity in patients with dilated cardiomyopathy.
