摘要
目的探讨皮下注射低分子肝素后局部压迫时间的长短与皮下出血发生率及出血面积的关系,以期找到合适的压迫时间。方法选取我院2013年5月至7月期间收治的下肢深静脉血栓患者为研究对象,共90例,其中60例采用三角肌皮下注射,30例采用腹壁注射。将每例患者的14次皮下注射根据局部压迫时间的长短每2次记为1组,每组压迫时间分别为:1、2、3、4、5、6、7 min。12 h后统计记录注射局部的皮下出血情况。结果压迫时间与出血率的关系比较:随着压迫时间的延长,两组的皮下出血率均降低;压迫时间为4 min组和5 min组比较有统计学差异(P<0.05),其他相邻组比较无统计学意义。压迫时间与出血面积的关系比较:压迫时间为3 min组和4 min组比较有统计学差异(P<0.05),其他相邻组比较无统计学意义。结论皮下注射低分子肝素后局部压迫5 min,可以显著降低皮下出血和大出血的发生率。
Objective To investigate the relationship between local compression time and the incidence and size of subcutaneous hemorrhage after low molecular weight heparin subcutaneous injection, in order to find a suitable compression time. Methods 90 cases of patients with deep vein thrombosis treated in our hospital from May 2013 to July 2013 were selected. 60 cases were given deltoid subcutaneous injection, while 30 cases were given abdominal wall injection. Every second of patients＇ 14 subcutaneous injection was taken as a group according to the length of local compression time. The compression time of every group was 1, 2, 3, 4, 5, 6, 7 min. The condition of subcutaneous hemorrhage at the injection site after 12 h was recorded. Results The relationship between time and bleeding rate was that with the extension of the compression time, subcutaneous bleeding rates of the two groups decreased; compression time of 4 min and 5 min group had statistically significant difference （P〈0.05）, other adjacent group hadn＇t. The relationship between compression time and bleeding size was that compression time of 4 rain and 5 min group had statistically significant difference （P 〈0.05）, other adjacent group hadn＇t. Conclusions Local oppressed 5 min after low molecular weight heparin subcutaneous injection can significantly reduce the incidence of subcutaneous hemorrhage and massive hemorrhage.
