摘要
目的探讨2μm铥激光尿道内切开治疗男性尿道狭窄及尿道闭锁的手术方法。方法使用德国LISA公司RevoLix 21μm连续波医用铥激光手术治疗系统，最大功率40w，行手术治疗男性尿道狭窄17例，男性尿道闭锁6例。手术均在尿道镜直视下配以寻光法引导进行。后续治疗采用传统的尿道扩张方法，每周1次尿道扩张，连续10次以上，之后每月1次尿道扩张持续1年以上。结果平均手术时间52（35～70）min，术后4—8周拔除导尿管，患者均排尿通畅，术后配合尿道扩张巩固疗效。1例患者术后4个月时尿道扩张困难，再次行经尿道狭窄激光内切开术，术后排尿通畅。结论2μm铥激光尿道内切开术治疗尿道狭窄及尿道闭锁操作简便．创伤小。并发症少。疗效可靠日．安全性高。
Objective To explore the 2μm thulium laser technique in the urethral surgical treatment for male urethrostenosis and urethratresia. Method RevoLix 2μm continuous wave medical laser surgery system （ LISA laser products） was used with maximum power of 40 W for the surgical treatment of 17 urethrostenosis ca- ses and 6 urethratresia cases. The operation was undergone under the direct field of urethrascope and the guidance of light transmission. Subsequent treatment still adopted the traditional method of urethral expansion, which was more than 10 consecutive weekly urethra expansion followed by an expansion monthly for more than one year. Results The operation time was 35 - 70 minutes with an average of 52 minutes. After 4 ~ 8 weeks urethral cath- eter insertion, the patients had good voiding, but postoperative urethral expansion was needed to consolidate cura- tive effect. 1 patient failed to do the postoperative urethral expansion 4 months after operation, and received the urethral stricture segment discussion again. Conclusion It is recommended of the 2μm thulium laser for the treatment of urethrostenosis and urethratresia with the advantages of easy manipulation, little trauma, few compli- cation, reliable effect and high safety.
