摘要
目的观察增强型体外反搏(enhanced external counterpulsation,EECP)治疗老年慢性收缩性心力衰竭患者的临床疗效。方法将40例老年慢性收缩性心力衰竭患者按随机数字表法分为两组各20例,两组均给予常规药物治疗,反搏组在此基础上加用EECP治疗。监测两组治疗前后6分钟步行距离、治疗3月后血浆NT-proBNP、左室舒张末期内径(LVEED)、左室射血分数(LVEF)值以及反搏组治疗前、首次EECP后2 h和24 h血浆NT-proBNP。结果反搏组首次EECP后2 h血浆NT-proBNP含量较治疗前升高(P<0.05),24 h后降至基线水平;治疗3月后反搏组血浆NT-proBNP低于对照组(P<0.05),6分钟步行距离、LVEF值明显高于对照组(P<0.05),而LVEED与对照组无明显差异。结论老年慢性收缩性心力衰竭患者EECP后即刻的血浆NT-proBNP水平升高,长期EECP治疗可使血浆NT-proBNP水平下降,能有效改善左室收缩功能和活动耐量。
Objective To observe the clinical efficacy of the treatment of elderly patients with chronic systolic heart failure by enhanced external counterpulsation(EECP).Methods Forty elderly patients with chronic systolic heart failure were randomly divided into two groups.The EECP group received a conventional drug therapy plus EECP treatment while the control group received a conventional drug therapy only.Six-minute walk distance,plasma concentrations of NT-proBNP,left ventricular end-diastolic diameter(LVEED) and left ventricular ejection fraction(LVEF) were measured before and after 3 months of treatment in the both groups.Results The plasma concentrations of NT-proBNP were increased immediately after 2h of treatment in the EECP group when compared to pretreatment,and NT-proBNP was decreased to baseline levels after 24 h.After 3 months of the treatment,the plasma concentration of NTproBNP in the EECP group was lower than that in the control group(P 〈0.05).The 6-minute walk distance and LVEF value in the EECP group were higher than that in the control group(P 〈0.05) while no significant difference in the LVEED was found between the two groups.Conclusion The plasma concentrations of NT-proBNP in elderly patients with chronic systolic heart failure were increased immediately after EECP treatment.Long-term EECP treatment could decrease the plasma NT-proBNP levels and improved the left ventricular systolic function as well as exercise tolerance effectively in elderly patients with chronic systolic heart failure.
