大肠癌中Fas/FasL、p27及PCNA的表达及意义

EXPRESSION AND SIGNIFICANCE OF FAS/FASL, P27 AND PCNA IN COLORECTAL NEOPLASM

摘要 目的 研究Fas/FasL在大肠癌及癌周不同部位细胞中的表达以及 p2 7、PCNA在大肠癌组织中的表达 ,并探讨其生物学意义。方法 利用免疫组化法对 50例大肠癌组织、癌旁交界组织、距肿瘤边缘 5cm处、正常组织的Fas/FasL表达进行检测 ,检验上述组织中Fas/FasL表达的差异。并对其中正常组织和肿瘤组织的 p2 7、PCNA在表达进行检测。 结果 比较肿瘤组织、癌旁交界组织与正常组织Fas的阳性表达率 (62 %vs84%vs1 0 0 % )和FasL的阳性表达率 (86 %vs66 %vs2 % ) ,差异均有显著性 (P <0 .0 5)。距肿瘤边缘5cm处与正常组织的Fas阳性表达率 (98%vs1 0 0 % )和FasL的阳性表达率 (8%vs2 % ) ,差异无显著性 (P >0 .0 5)。而不同分化的肿瘤组织中Fas/FasL表达差异无显著性 (P >0 .0 5)。p2 7在肿瘤组织与正常组织中的高表达率分别为 54 %和 1 0 0 % ;PCNA的阳性细胞数分别为 (2 2 .37± 6 .0 5)和 (50 .91± 1 6 .0 6) ,PCNA和p2 7在二组织中的表达差异均有显著性 (P <0 .0 5) ,且 p2 7、PCNA表达与大肠癌的分化程度、淋巴结转移和Dukes分期显著有关。结论 大肠癌中存在Fas表达的下调和FasL表达的上调 ,Fas/FasL系统可能参与了大肠癌细胞的免疫逃避机制 ;p2 7蛋白的表达与大肠癌的临床病理学特征密切相关 ,可能是一个? Objective To study the expression of Fas/FasL in different location of colorectal tissue and the expression of p27 and PCNA in colorectal neoplasm. Methods 50 consecutive colorectal neoplam specimens were collected and continuous parafin slices were made from the normal colorectal tissue, 5cm away from colorectal neoplasm tissue, paracarcinoma tissue and colorectal neoplasm tissue of each specimen. The expressions of Fas/FasL were analyzed with immunohistochemistry in order to find the difference in the above-mentioned tissues. Immunohistochemistry staining was also used to detect p27 and PCNA in the above-mentioned normal colorectal tissue and colorectal neoplasm tissue. Results In comparison, the positive rates of Fas expressions in the colorectal neoplasm tissue, paracarcinoma tissue and normal colorectal tissue (62% vs 84% vs 100%) showed significant difference (P<0.05) from those of FasL expressions (86% vs 66% vs 2%). In comparison between the normal colorectal tissue and the tissue 5cm away from the colorectal neoplasm tissue, the Fas expression positive rates (98% vs 100%) and the FasL expression positive rates (8% vs 2%) showed no significant difference (P>0.05). There was no significant difference in the expression of Fas/ FasL in carcinoma with different differentiation grades. P27 high expression rate for normal tissue was 100%, but for colorectal neoplasm tissue that was 54%; the number of PCNA positive cells in normal tissue and colorectal neoplasm tissue was 22.37±6.05 and 50.91±16.06 respectively. They also showed significant difference (P<0.05) in normal tissue and colorectal neoplasm tissue. In colon cancer the expression of PCNA and p27 was correlated with the degree of differentiation, stage and lymphatic metastasis. Conclusion Down-regulation of Fas and up-regulation of FasL are early and frequent events associated with the evolution of colorectal neoplasm. The expression of p27 in colorectal neoplasm is significantly associated with clinicopathological characteristics and it may be an important factor to the prognosis of colorectal neoplasm. PCNA is an important marker to reflect cell proliferation.
