摘要
目的 比较腰麻－硬膜外联合麻醉和单纯硬膜外麻醉用于妇科腹腔镜手术对BP、HR、SPO2、PETCO2及麻醉效果、气腹充气量的影响。方法50例妇科腹腔镜手术病人随机分为腰麻－硬膜外联合麻醉组(CSEA组,n=25例)和单纯硬膜外麻醉组(EA组,n=25例),监测患者于麻醉前、气腹前、气腹 10 min、30min及解除气腹后 10 min,5个时间点HR、BP、SPO2和 PETCO2的变化,记录腹内压达1．17 kPa时的气腹充气量及麻醉效果。结果①气腹后HR、BP、PETCO2均明显变化(P ＜0．05),EA组阶变化比CSEA组更明显(P＜0．05);②CSEA组的气腹充气量明显多于EA组(P＜0．05);③CSEA组的麻醉效果优于 EA组(P ＜0．05)。结论 腰麻－硬膜外联合麻醉用于妇科腹腔镜手术对循环影响小,麻醉效果优于单纯硬膜外麻醉,是较理想的麻醉方法。
ve To compare the effect and pneumascos aeration of combined spinal -epidural anesthesia with peridural anesthesia laparascopic gynecology surgery on HR, BP, SpO2, PETCO2 anesthetic effect and the volume of CO2 insufflation when intra-abdominal pressure reaches 1. 17KPa. Methods Fifty patients undergoing laparoscopic gynecology surgery were divided into combined spinal -epidural anesthesia group (group CSEA, n =25) and peridural anthesia group (group EA , n =25), HR, BP, SpO2 and PETCO2 were monitored at five time points as follows : before anesthesia, before insufflation, at 10min, 30min after insufflation respectively, and after desufflation. The volume of CO2 insufflation and the anesthetic effect were recorded. Result ①HR, BP and PETCO2 had increaseed significantly ( P <0. 05) after pneumascos in both two groups, the increase of BP in group EA were significantly higher than in group CSEA.②The volume of CO2 insufflation in group CSEA was significantly more than in group EA. ③The anesthetic effect of group CSEA was significantly better than group EA. Conclusion The combined spinal-epidural anesthesia may alleviate circulation, gain better anesthetic effect and be an ideal technique for laparoscopic gynecology surgery .
