摘要
目的分析徐州市第一人民医院2015—2017年住院患者抗菌药物使用情况。方法通过PASS临床药学管理系统及医院信息管理系统(HIS)调取徐州市第一人民医院2015—2017年住院患者抗菌药物使用情况,记录其抗菌类药物使用率、使用强度(AUD)及本院抗菌类药物AUD、排名,并分析AUD排名前10的抗菌药物及各科室AUD、排名。结果 2015—2017年本院住院患者抗菌类药物使用率分别为63.36%、59.69%、51.18%,AUD分别为64.11、56.14和41.20。2015—2017年本院AUD排名前4的抗菌类药物分别为喹诺酮类、头孢霉素类、三代头孢菌素和青霉素类+酶抑制剂。连续3年排在前10的抗菌药物分别为头孢西丁、左氧氟沙星、头孢地嗪、阿莫西林克拉维酸钾、头孢吡肟、头孢哌酮舒巴坦、头孢呋辛钠和哌拉西林他唑巴坦。2015—2017年本院各科室AUD排名前5名:2015年为呼吸科、泌尿外科、ICU、普外科,消化科;2016年为呼吸科、普外科、耳鼻喉科、泌尿外科、消化科;2017年为呼吸科、ICU、普外科、儿科、耳鼻喉科。结论本院对抗菌药物合理使用管理有效,通过采取措施得当,临床用药品种选择的合理性及各科室抗菌药物的使用合理性得到一定改善,但仍存在不合理用药现象,有待进一步加强管理,确保临床用药安全。
Objective To analysis the antibiotics use in inpatients of Xuzhou First People’s Hospital from 2015 to 2017.Methods The use of antibiotics in inpatients of Xuzhou First People’s Hospital from 2015 to 2017 were collected through PASS clinical pharmacy management system and hospital information management system(HIS).Recorded the use rate and intensity of antimicrobial drugs(AUD) in hospitalized patients from 2015 to 2017,the AUD and ranking of antimicrobial drugs in our hospital from 2015 to 2017,the top 10 antimicrobial drugs in our hospital from 2015 to 2017,and the AUD and ranking of various departments in our hospital from 2015 to 2017.Results From 2015 to 2017,the utilization rates of antibiotics in our hospital were 63.36%,59.69%,51.18%,and the AUD were 64.11,56.14 and 41.20 respectively,showed a decreasing trend year by year.The top 4 AUD antibiotics in our hospital from 2015 to 2017 were quinolones,cephalosporins,third generation cephalosporins and penicillins+enzyme inhibitors.Cefoxitin,levofloxacin,cefodizine,amoxicillin,clavulanate potassium,cefepime,cefoperazone sulbactam,cefuroxime sodium and piperacillin tazobactam were the top 10 antimicrobial drugs for three consecutive years.From 2015 to 2017,AUD ranked the top five departments in our hospital:Respiratory,Urology,ICU,General Surgery,Digestive Department in 2015;Respiratory,General Surgery,Otolaryngology,Urology,Digestive Department in 2016;Respiratory,ICU,General Surgery,Pediatrics,Otolaryngology in 2017.Conclusion The rational use of antimicrobial drugs in our hospital is effective.Appropriate measures have been taken to improve the rationality of clinical drug use and the rationality of antimicrobial drug use in various departments.However,irrational use of antimicrobial drugs still exists,which need to be further strengthened to ensure the safety of drug use.
