摘要
从踝关节生物力学的角度综述膝骨关节炎(KOA)的临床研究进展。通过整理国内外文献,概述KOA发生及发展过程中下肢运动链(髋-膝-踝)的生物力学机制。在步行活动中,踝关节是膝与地面作用的中间环节,膝-踝关节间力学联系紧密; KOA可造成踝关节结构和功能异常,踝关节损伤或功能下降亦可诱导或加重KOA。基于踝关节生物力学特点,临床治疗采用足部矫形器、改变足前进角、中医手法和太极拳等康复疗法,取得一定疗效。
This article reviews the clinical research progress of knee osteoarthritis( KOA) from the perspective of ankle joint biomechanics. By collecting the domestic and foreign literatures,the biomechanics mechanisms of lower limbs kinematic chain( hip-kneeankle) during the occurrence and development of KOA are outlined. In the walking activity,the ankle joint is the middle link of interaction between the knee and the ground,and there is closely mechanical connection between the knee and the ankle. KOA can cause abnormalities in structure and function of ankle joint,and ankle joint damage or functional decline can also induce or aggravate KOA.Based on the biomechanical characteristics of ankle joint,in clinical treatment,rehabilitation therapies such as foot orthosis,change of foot forward angle,Chinese medical manipulation and Taijiquan are adopted,which obtains certain efficacy.
