微创钢板接骨术联合高压氧治疗成人胫骨骨折的疗效研究

摘要 目的:探讨高压氧辅助微创钢板接骨术对胫骨骨折的疗效及对血清胰岛素样生长因子-1(IGF-1)和成纤维细胞 生长因子-2(FGF-2)水平的影响。方法:选取本院收治胫骨骨折(开放性骨折)患者82例,参照数字表法随机分为对照组和观 察组,每组均41例。对照组行微创钢板接骨术。观察组在对:照组基础上于术后1天采取高压氧干预,每天1次,连续4周。比较 2组术后疼痛消失时间、骨折愈合时间、骨折区骨密度及临床疗效。检测2组血清IGF-1和FGF-2水平。结果:观察组患者的 疼痛消失、骨折愈合时间显著少于对照组(P<0.01)。术后6个月、12个月,观察组患者的骨密度水平显著高于对照组同期(P<0.01)。观察组优良率为97.56%,明显高于对照组的80.49%(x^2=4.493,?<0.05)。术后4周,观察组患者血清IGF-1和FGF-2水平明显高于对照组(P <0.01)。结论:高压氧辅助微创钢板接骨术治疗胫骨骨折的疗效确切,上调患者血清IGF-1和FGF-2水平可能是其疗效途径之一。 Objective:To observe efficacy of high pressure oxygen assisting Minimally invasive plate osteosynthesis in treating tibial fracture,and its effect on serum levels of insulin-like growth factor-1(IGF-1)and fibroblast growth factor-2(FGF-2).Methods:Eighty two cases with tibial fracture were selected and randomly divided into control group(41 cases)and observation group(41 cases).Control group was given with Minimally invasive plate osteosynthesis.Based on control group,observation group was treated with high pressure oxygen postoperative 1 day for 4 weeks.Pain disappearance time,fracture healing time,fracture area bone mineral density,and efficacy were compared in two groups.Serum levels of IGF-1 and FGF-2 were detected in both groups.Results:Pain disappearance time and fracture healing time of observation group were obviously less than control group(P<0.01).6 and 12 months after operation,fracture area bone mineral density of observation group was remarkably higher than control group at the same time(P<0.01).Excellent rate of observation group was 97.56%,which was obviously higher than that of control group 80.49%(P<0.05).4 weeks after operation,serum levels of IGF-1 and FGF-2 of observation group were evidently higher than those of control group(P<0.01).Conclusion:Efficacy of high pressure oxygen assisting Minimally invasive plate osteosynthesis in treating tibial fracture,which was related to regulating serum levels of IGF-1 and FGF-2.
