T-SPOT.TB与TB-Ab检测在小儿结核病诊断中的应用 被引量:1

Application of T-cell spot test and anti-tuberculosis antibody test in the diagnosis of children tuberculosis

摘要 目的评价结核感染T细胞试验(T-SPOT.TB)及抗结核抗体(TB-Ab)检测在小儿结核病中的应用价值。方法回顾性分析193例患儿两种方法的检测结果,比较T-SPOT.TB与TB-Ab的检出阳性率、灵敏度及特异性;并对T-SPOT.TB在肺结核组与肺外结核组的斑点计数进行比较。结果T-SPOT.TB在肺结核组、肺外结核组的检出阳性率(93.15%、80.00%)明显高于TB-Ab(39.73%、30.91%),T-SPOT.TB的灵敏度和特异性(87.50%、92.31%)均优于TB-Ab(35.94%、78.46%),差异均有统计学意义(P<0.05)。对肺结核组与肺外结核组斑点的数量进行比较,差异无统计学意义(P>0.05)。结论与TB-Ab相比,T-SPOT.TB检出阳性率高、灵敏度高、特异性强,更适用于小儿结核病的辅助诊断;初步研究提示,斑点数量与结核组别没有相关性。 Objective T o evaluate the clinical value of T-cell spot test(T-SPOT.TB)and anti-tuberculosis antibody test(TB-Ab) in children with tuberculosis. Methods The positive rate,sensitivity and specificity of T-SPOT. TB and TB-Ab were compared by retrospectively analyzing the diagnosis results of193 patients;spot counting of T-SPOT.TB was also compared between pulmonary tuberculosis group and extra pulmonary tuberculosis group. Results The positive rate of the T-SPOT.TB in tuberculosis and extra-pulmonary tuberculosis groups(93.15%,80.00%) was significantly higher than that of TB-Ab(39.73%,30.91%). The sensitivity and specificity of T-SPOT.TB(87.50%,92.31%)were also better than the TB-Ab(35.94%,78.46%). The differences above were statistically significant(P <0.05). No statistically significant difference was found in the number of spots between the pulmonary tuberculosis and the extra pulmonary tuberculosis group(P >0.05). Conclusion Compared with the TB-Ab,the T-SPOT.TB had higher positive rate,sensitivity and specificity,and was more suitable for the auxiliary diagnosis of tuberculosis in children;however,preliminary studies suggest that there is no correlation between the number of spots and the group of tuberculosis.
