摘要
目的探讨超声探测技术联合T淋巴细胞水平在11-14w孕妇胎儿畸形中的筛查效果及与预后的相关性分析。方法选择2015年6月-2017年10月入院进行胎儿畸形筛查孕妇16793例,先行常规二维超声检查,对于疑似畸形胎儿采用表面三维超声检查。对于疑似畸形胎儿在征求家属同意后在孕晚期对产妇进行T淋巴细胞水平CD3+、CD4+、CD8+及CD4+/CD8+水平(流式细胞仪)测定;采用SPSS Pearson相关性分析软件对超声探测技术联合T淋巴细胞水平筛查结果与预后进行相关性分析。结果 16793例产妇中最终确诊186例胎儿畸形,畸形率为1.11%。疑似畸形胎儿孕妇CD3+、CD4+及CD4+/CD8+水平,均低于正常孕妇(P<0.05)SPSS Pearson相关性分析结果表明:畸形胎儿预后与超声检查结果呈正相关性(P<0.05);畸形胎儿预后与CD3+、CD4+及CD4+/CD8+水平呈负相关性(P<0.05)。结论将超声探测技术联合T淋巴细胞水平用于11-14w孕妇胎儿畸形筛查中能提高筛查符合率。
Objective:To explore the screening effect of ultrasound detection technology combined with T-lymphocyte levels in fetal malformations of 11-14 weeks pregnant women and its correlation with prognosis. Methods:A total of 16 793 pregnant women admitted to the hospital from June 2015 to October 2017 were selected for fetal malformation screening. Conventional twodimensional ultrasonography was performed first. Three-dimensional ultrasonography was performed on suspected fetuses. For the fetus with suspected malformation,after the consent of the family members,the T-lymphocyte levels of CD3+,CD4+,CD8+,and CD4+/CD8+ levels(flow cytometry)were measured in the third trimester;the SPSS Pearson correlation analysis software was used for the ultrasound detection technology. The correlation between lymphocyte level screening results and prognosis was analyzed. Results:A total of 186 cases of fetal malformations were diagnosed in 16 793 women. The malformation rate was 1.11%. The levels of CD3+,CD4+,and CD4+/CD8+ in pregnant women with suspected malformations were all lower than those in normal pregnant women(P<0.05). The Pearson correlation analysis of SPSS showed that the prognosis of malformed fetuses was positively correlated with ultrasound findings(P<0.05);the prognosis of malformed fetuses There was a negative correlation between CD3+,CD4+,and CD4+/CD8+ levels(P<0.05). Conclusions:The use of ultrasound detection combined with T-lymphocyte levels for fetal malformation screening in 11-14 weeks pregnant women can improve the screening rate.
