摘要
目的提高日间化疗中心肿瘤患者补充性肠外营养(SPN)医嘱合理性,为临床提供参考。方法统计某院2017年10月～12月日间化疗中心388例患者共631份SPN医嘱,对其进行分析。结果 SPN医嘱在液体量、总热量、糖脂比、阳离子与渗透压等方面使用基本合理,但部分医嘱在使用天数、非蛋白热氮比、维生素及微量元素使用比例等方面仍存在不合理之处。结论临床医生应准确评定患者营养状况,个体化设计配方。药师应强化审方意识并加强与临床沟通,进一步提高合理应用SPN水平。
OBJECTIVE To improve the rationality of supplementary enteral nutrition (SPN) prescriptions in day-time chemotherapy center, and provide reference for clinic. METHODS A total of 631 SPN medical prescriptions were analyzed in 388 patients received by the day-time chemotherapy center of our hospital from October to December of 2017. RESULTS It was basic reasonable of liquid volume, total caloric, sugar lipid ratio, cationic, osmotic pressure, but there was still no specification such as the short of using day, the slightly low of the non-protein caloric nitrogen ratio, the insufficient proportion of using the vitamins and trace elements, etc. CONCLUSION The doctors should assess the nutritional status accurately and individualize the SPN formula. The pharmacist should strengthen the consciousness of the prescription, and strengthen the communication with the clinical staff to further improve the rationality of SPN prescriptions.
