摘要
目的:回顾性分析InterTAN和PFNA治疗老年性股骨粗隆间骨折的隐性失血量,为临床选择髓内固定治疗提供参考。方法:回顾性分析2016年3月至2017年6月在该院行InterTAN手术治疗的22例和采用PFNA治疗的29例老年性股骨粗隆间骨折的临床资料。采集两组患者术前、术后血常规,手术时间,术中、术后出血量和输血情况,评估患者围手术期的隐性失血和总失血量。结果:InterTAN组的手术时间(79.8±10.7)min,术中出血量(181.5±60.8)mL,术后失血量(70.5±20.2)mL,隐性失血量(487.6±99.5)mL,总失血量(790.2±115.6)mL,PFNA组的手术时间(66.2±11.8)min,术中出血量(122.9±51.2)mL,术后失血量(66.8±22.1)mL,隐性失血量(473.5±87.9)mL,总失血量(692.1±104.9)mL,InterTAN组手术时间较PFNA组长(P<0.05),术中失血量和总失血量也较PFNA组高(P<0.05),但两组的术后出血量和隐性失血量无明显统计学差异(P>0.05)。结论:InterTAN和PFNA髓内固定系统均存在隐性失血量,术前、术中注意匹配度,注意围手术期患者的临床生命体征以减少并发症的发生。
Objective: To analyze hidden blood loss in femoral inter-trochanteric fracture elder patients treated with InterTAN and PFNA nails retrospectively,and to provide a refernce for inntramedullary fixation in clinic.Methods: The clinical data of 22 cases of inter-trochanter fracture elderly patients treated with InterTAN nails and 29 cases of intertrochanter fracture elderly patients treated with PFNA nails fr om Mar.2016 to Jun.2017,and the results of blood routine examination before and after operation,operation time ,obvious blood loss in and after operation and blood transfusion was retrospectively analyzed,and then hidden blood loss and total blood loss in these patients per-operation was to evaluate.Rusults: The data of InterTAN group: the operation time was(79.8±10.7)min,the obvious blood loss in operation was(181.5±60.8)mL,the obvious blood loss was(70.5±20.2)mL,the hidden blood loss was(487.6±99.5)mL,the total blood loss was(790.2±115.6)mL.The datas of PFNA group: the operation time was(66.2±11.8)min,the obvious blood loss in operation was(122.9±51.2)mL,the obvious blood loss was(66.8±22.1)mL,the hidden blood loss was(473.5±87.9)mL,the total blood loss was(692.1±104.9)mL.The operation time in Inter-TAN group was longer than that of PFNA group(P<0.05),and the obvious blood loss in and after operation was also larger than that of PFNA(P<0.05).While,there was no difference in hidden blood loss and total blood loss between the two groups with statistic significance(P>0.05).Conclusion: There was much hidden blood loss in femoral intertrochanteric fracture elder patients treated with InterTAN and PFNA nails,it reminds surgeons of implement of matching- degree of inntramedullary fixation before and in operation and monitoring the vital signs per-operation to decrease the complications.
