摘要
目的了解临床分离的固有耐药细菌的感染分布情况及药敏结果,为临床治疗合理使用抗生素提供理论依据。方法收集2016年10月-2017年9月本院住院患者标本中检测出的固有耐药菌,采用美国BD公司Phoenix-100全自动微生物仪进行细菌鉴定及药敏分析。结果检出的1 637株固有耐药细菌中主要有铜绿假单胞菌624株(38. 1%);鲍曼不动杆菌498株(30. 4%);阴沟肠杆菌250株(15. 3%)。主要标本类型为痰液1 133株(69. 2%),其次是创面分泌物304株(18. 6%)。阴沟肠杆菌、黏质沙雷菌、铜绿假单胞菌对氨曲南的耐药率分别为21. 6%、22. 5%、23. 7%;对美罗培南的耐药率分别为3. 2%、22. 5%、17. 3%;对亚胺培南的耐药率分别为3. 2%、22. 5%、22. 0%。阴沟肠杆菌对头孢吡肟、庆大霉素、阿米卡星的耐药率分别为13. 2%、1. 6%、6. 0%。结论固有耐药细菌感染分布广,临床治疗应严把用药适应症,合理选择抗生素,防止耐药性的日渐增加。
Objective To understand the infection distribution and drug susceptibility results of clinically isolated intrinsically resistant bacteria,so as to provide basis for the rational use of antibiotics in clinical treatment. Methods The intrinsic drug-resistant bacteria detected in the inpatient samples from October 2016 to September 2017 were collected. The bacteria identification and drug sensitivity analysis were performed using the American BD Phoenix-100 automatic microbiological instrument.Results There were mainly 624 strains of Pseudomonas aeruginosa( 38. 1%),498 strains of Acinetobacter baumannii( 30. 4%),250 strains of Enterobacter cloacae( 15. 3%) among the 1 637 resistant tested strains. The main specimen type was1 133 sputum( 69. 2%),followed by wound secretion 304( 18. 6%). The resistance rates of Enterobacter cloacae,Serratia marcescens,Pseudomonas aeruginosa were respectively 21. 6%,22. 5%,and 23. 7% to aztreonam,3. 2%,22. 5% and17. 3% to meropenem,and 3. 2%,22. 5%,and 22. 0% to imipenem. The resistance rates of Enterobacter cloacae to Cefepime,Gentamicin and Amikacin were respectively 13. 2%,1. 6% and 6. 0%. Conclusion Inherently resistant bacterial infections are widely distributed,so clinical treatment should be strict on medication indications,and adopt reasonable antibiotics selection,so as to prevent the increasing of drug resistance.
