摘要
目的:观察防己黄芪汤加减治疗射血分数保留型心衰(HFPEF)心肺气虚证的疗效及对血清可溶性ST2(sST2)和半乳糖凝集素(gal)-3水平的影响。方法:将74例HFPEF(心肺气虚证)患者按数字表法随机分为对照组和治疗组,每组37例。对照组依据《射血分数正常心力衰竭诊治的中国专家共识》予常规干预和对症治疗。观察组在对照组基础上予防己黄芪汤加减治疗,1剂/d,常规水煎分2次温服。两组连续治疗2周。比较两组心肺气虚证积分、两组6 min步行实验(6 MWT)及临床疗效。检测两组血清B型利钠肽(BNP)、sST2和gal-3水平。结果:治疗后,观察组患者心肺气虚证症状评分、血清BNP水平显著低于对照组,6 MWT明显高于对照组(P<0.01)。观察组总有效率为91.89%,明显高于对照组70.27%(P<0.05)。治疗后,观察组血清sST2和gal-3水平明显低于对照组(P<0.01)。结论:在常规西医基础上,防己黄芪汤加减HFPEF心肺气虚证疗效确切,且可下调患者体内sST2和gal-3水平。
Objective:To observe the efficacy of Fangji Huangqi Decoction in treating heart failure with preserved ejection fraction(HFPEF) with syndrome of Qi deficiency of heart and lung and its influence on serum levels of soluble ST2(sST2) and galactose agglutinin(gal)-3.Methods:Seventy-four cases with heart failure with preserved ejection fraction(syndrome of Qi deficiency of heart and lung) were randomly assigned to control group and observation group with 37 cases in each according to random digits table.Control group was given routine and symptomatic treatment referring to Chinese Experts Agree on The Diagnosis and Treatment of Normal Heart Failure.Based on control group,observation group were treated with Fangji Huangqi Decoction(1 dose/d,bid).Treatment course was 2 weeks for both groups.Symptoms scores of syndrome of Qi deficiency of heart and lung,6 minutes walk test(6 MWT) and efficacy were compared in both groups.Serum levels of B-type natriuretic peptide(BNP),sST2,and gal-3 were detected in both groups.Results:Symptoms scores of syndrome of Qi deficiency of heart and lung and serum level of BNP of observation group were obviously lower,while 6 MWT was higher than that of control group(P<0.01).Total effective rate for observation group was 91.89%,which was superior to control group’s 70.27%(P<0.05).After treatment,serum levels of sST2 and gal-3 of observation group were evidently lower than those of control group(P<0.01).Conclusion: On the basis of conventional western medicine,the efficacy of Fangji Huangqi Decoction in treating HFPEF with syndrome of Qi deficiency of heart and lung is significant and Fangji Huangqi Decoction can down-regulate serum levels of sST2 and gal-3.
