摘要
目的了解广西农村老年人性行为及其影响因素,为开展针对安全性行为、艾滋病性病的宣传教育和干预措施提供科学依据。方法 2016年8月至2017年3月,在广西艾滋病疫情较为严峻的某县2个乡镇,对≥50岁并在半年内发生过性行为的当地居民进行问卷调查和血清学检测,采用χ~2检验和多因素Logistic回归进行统计分析。结果共调查553名≥50岁老年人。其中26.22%的老年人曾发生临时性行为。临时性伴组近1个月内发生过性行为的比例(48/145,33.10%)低于固定性伴组(238/408,58.33%),但每月发生8次以上性行为(4.83%)和每次发生性行为都使用安全套(11.03%)的比例均高于固定性伴组(1.23%、3.19%),差异均有统计学意义(P均<0.05)。在男性调查对象中,临时性伴组和固定性伴组在使用壮阳药、愿意暴露前用药和愿意做包皮环切术的情况均无统计学差异(P均>0.05)。多因素分析结果显示,未婚/离异/丧偶[比值比(OR)=6.24,95%可信区间(CI):2.75~14.20,P<0.05]是发生临时性行为的影响因素;年龄≥60岁(OR=1.90,95%CI:1.06~3.40,P<0.05)、未婚/离异/丧偶(OR=6.56,95%CI:2.49~17.28,P<0.05)是男性发生临时性行为的影响因素。临时性伴组的HIV抗体阳性率(4.14%)高于固定性伴组(0.49%,P<0.05)。结论未婚/离异/丧偶是发生临时性行为的危险因素,发生临时性行为可以增加感染HIV的风险。应加强农村老年人艾滋病性病的宣传教育和干预,减少不安全性行为的发生。
Objective To understand the sexual behavior and its influencing factors among elderly people in the rural area of Guangxi and provide scientific basis for the targeted education and intervention of safe sexual behavior, HIV/AIDS and sexually transmitted diseases.Methods From August 2016 to March 2017,a questionnaire survey with serological test was conducted among 50 years old people in two towns with higher HIV/AIDS epidemic.Statistics analysis was performed by chi-square test and multivariate logistic regression.Results Among the 553 elderly people surveyed,26.22% had sexual behavior with temporary partners,and 33.11% had sexual behavior in the last month,which was lower than that of those who had regular sexual partners (58.33%).4.83% of those with temporary partners had sexual behavior more than 8 times a month and used.condoms every time (11.03%),which was higher than the group with regular sexual partner (1.23%,3.19%),and the differences were statistically significant (P<0.05).Among the male elderly people,there was no difference between the two groups in use of aphrodisiacs,willingness to use pre-exposure prophylaxis,and willingness to receive circumcision (P>0.05).Multivariate analyses showed that unmarried/divorced/widowed (OR=6.24,95% CI=2.75-14.20,P<0.05)was the influencing factor of temporary sexual behavior.Furthermore,60years and older (OR=1.90,95%CI=1.06-3.40,P<0.05) and unmarried/divorced/widowed (OR=6.56,95% CI=2.49-17.28,P<0.05)were the influencing factor of temporary sexual behavior for males.The HIV positive rate in the group with temporary sexual partners (4.14%)was higher than that in the one with regular sexual partners (0.49%,P<0.05).Conclusion Unmarried/divorced/wid-owed is the risk factor of temporary sexual behavior.Temporary sexual behavior can increase the risks of HIV infection.The education and intervention of HIV/AIDS and sexually transmitted diseases among middle-aged and elderly people in rural areas should be enhanced to reduce the occurrence of unsafe sex.
