摘要
目的 报道低体重婴儿室间隔缺损 (VSD)伴肺动脉高压的外科手术修补结果和体会。方法 1994年 3月至 1999年 12月 ,对 5 0例 2～ 11月龄 ,体重 3 2～ 5 0kg的VSD病婴采用中度低温体外循环行补片修补术。全组术前均伴中度以上肺动脉高压 (Pp/Ps平均为 0 6 8± 0 0 9)。结果 住院死亡 2例 ,住院死亡率为 4% ,死亡原因分别为心包压塞和脑昏迷。术后主要并发症为肺部感染、肺动脉高压危象和心律失常。结论 对
Objective: To review the experience of surgical repair of ventricular septal defect (VSD) with pulmonary hypertension in infants weighing less than 5?kg. Methods: Fifty infants weighing less than 5?kg with VSD underwent surgical repair form March 1994 to December 1999. Teir body weights ranged from 3 2?kg to 5 0?kg (mean 4 4?kg). Their ages were 2 to 11 months (mean, 6 00?months). All patients had moderate or severe pulmonary hypertension before operation (Pp/Ps ranging from 0 50 to 0 94,mean 0 68). 44 patients had perimembranous VSD and 6 had subpulmonary VSD. All patients underwent surgical repair with moderate hypothermia CPB. Results: Two patients died during hospitalization with a hospital mortality of 4 0%. The leading complications were pulmonary infection, pulmonary hypertensive crisis and heart arrhythmia. Conclusion: Surgical treatment of VSD with pulmonary hypertension in infancy weighing less than 5kg may have a satisfactory outcome, with the standarized surgical techniques and the improved perioperative management.
