摘要
目的 观察激素性股骨头坏死生化指标的变化 ,探讨骨代谢变化在激素性股骨头坏死过程中的作用。方法 健康成年雄性新西兰大白兔 30只 ,随机分为模型组和对照组 ,采用臀肌注射大剂量醋酸氢化泼尼松进行造模。每隔 2周分别测定股骨头羟脯氨酸 (HOP)、氨基已糖 (HOM )、钙 (Ca)及血清钙、钙磷的含量 ,并计算出HOM /HOP、Ca/HOP的比值及血清钙磷乘积 ,同时观察股骨头X线和组织学的变化。结果 模型组羟脯氨酸、氨基已糖、钙含量及HOM/HOP、Ca/HOP比值均明显低于对照组 (P <0 0 5 ,P <0 0 1) ,血清钙、钙磷乘积亦明显低于对照组 (P <0 0 1,P <0 0 0 1) ;X线示模型组股骨头及周围骨小梁变稀疏 ;组织学显示模型组骨陷窝空缺数明显增多 ,骨小梁稀疏细长 ,髓腔内充满增大的脂肪细胞。结论 大剂量激素可引起股骨头骨细胞早期坏死 ,大剂量激素引起股骨头骨代谢变化是骨坏死综合因素之一。
Objective To study the mechanism of bone metabolism of steroid-induced necrosis of femoral head(SINFH).Methods Thirty male New Zealand rabbits were randomly divided into two groups:normal control group and model group.A model of SINFH was induce by intramuscularly hydrocortisone acetate overdose.The hydroxyproline(HOP),hexosamine(HOM) and calcium(Ca) of femoral head and serum-calcium, serum-phosphorous were measured and the ratio of HOM/HOP, Ca/HOP and the serum-calcium-phosphorous-product were calculated every two weeks, as well as the X-ray and histopathology of femoral head observed.Results It showed HOP, HOM, Ca and the ratio of HOM/HOP, Ca/HOP of femoral head of model group obviously lower than control group as well as the serum-calcium and serum-calcium-phosphorous-product were obviously lower than control grorp.The X-ray showed the model group became osteoporosis and the histopathology showed the ratio of empty lacunae and the diameters of fat cell in medullae of model group increased obviously(P<0.01),otherwise the diameters of trabecula decreased obviously. Conclusion Overdose steroid can induce early necrosis of femoral head.The alteration of bone metabolism caused by overdose steroid was one of the complex factors of SINFH.
