摘要
目的评估会阴按摩联合无保护接生技术的应用效果。方法选取2013年1~12月在本院住院的非高危自然分娩的初产妇200例,随机分为两组,对照组100例,接受传统接生技术,实验组100例,采用会阴按摩联合无保护接生技术。比较两组产妇自然分娩时的会阴情况、第二产程时间、产时和产后出血量及新生儿Apgar评分、是否有产瘤及头皮肿形成。结果实验组产妇的会阴完整率优于对照组,会阴侧切率及出血量低于对照组,差异有统计学意义（P〈0.05）。两组新生儿Apgar评分比较差异无统计学意义（P〉0.05）。实验组新生儿产瘤及头皮血肿的发生率低于对照组（P〈0.01及P〈0.05）。结论会阴按摩联合无保护接生技术能提高接产时会阴的完整性,降低会阴侧切率,缩短第二产程,减少产时和产后出血,对新生儿结局无不良影响,同时还可减少产瘤及新生儿头皮血肿的发生率。
Objective To evaluate the application effect of perineal massage combined with unprotected delivering technology. Methods 200 cases of high-risk natural birth maternal were selected in our hospital from January to December in 2013,they were randomly divided into two groups,control group of 100 cases accepted the traditional midwives technology,experimental group of 100 cases accepted the perineal massage combined with unprotected midwives technology.The perineum situation of natural childbirth,the second labor time,intrapartum and postpartum blood loss and neonatal Apgar score if there was a tumor and formed in the scalp of two groups were compared. Results The perineum complete rate in experimental group was better than that of control group,while episiotomy rate and haemorrhage amount was lower than the control group,the difference was statistically significant （P〈0.05）.Compared two groups of neonatal Apgar score,there was no statistically significant difference （P〉0.05）.The incidence of neonatal tumor producing and scalp hematoma in experimental group were lower than those of the control group （P〈0.01 and P〈0.05）. Conclusion Perineal massage combined with unprotected delivering technology can improve the integrity of the perineum when delivery,reduce episiotomy rate,shorten the labor,reduces intrapartum and postpartum hemorrhage,has no adverse effect on neonatal outcomes,and can also reduce maternal tumor and the incidence of neonatal scalp hematoma.
