摘要
目的 分析输尿管镜联合钬激光治疗输尿管结石的临床效果.方法 回顾性分析2012年2月～2014年2月笔者参与诊治250例输尿管结石患者的临床资料,随机选取对照组与实验组各125例,实验组给予输尿管镜联合钬激光规范治疗,对照组给予体外冲击波碎石术治疗,观察并分析两种方案的临床疗效.结果 实验组中,122例患者手术成功（成功率为97.6％）,全组共112例患者一次碎石成功（成功率为89.6％）;手术时间为（31.8±6.7） min;术后腰痛、发热、血尿发生率分别为8.8％、3.2％、45.6％.对照组中,120例患者手术成功（成功率为96.0％）,全组共101例患者一次碎石成功（成功率为80.8％）;手术时间为（40.2±3.4） min;术后腰痛、发热、血尿发生率分别为50.4％、13.6％、49.6％;实验组一次碎石成功率、手术时间、腰痛、发热等并发症发生率明显优于对照组,组间差异有统计学意义（P＜0.05）.结论 输尿管镜联合钬激光治疗输尿管结石的手术成功率、一次碎石成功率、治疗时间均理想,并发症少,安全性高,值得推广应用.
Objective To analyze the ureteroscope holmium laser application methods and clinical effects in the clini- cal treatment of ureteral stones. Methods 250 cases of patients with ureteral stones from February 2012 to February 2014 in our hospital were randomly divided into two groups：the control group and the＇experimental group,125 cases in each group.The experimental group were treated with ureteroscopy and holmium laser,while the control group were treated with extracorporeal shock wave lithotripsy.The clinical efficacy of the two schemes were analyzed. Results The experimental group,122 patients surgical success （success rate was 97.6%）,whole group of 112 patients with a gravel success （success rate was 89.6%）,mean operative time was （31.8±6.7） min,postoperative back pain,fever,hematuria were 8.8%,3.2% and 45.6%.The control group,120 patients surgical success （success rate was 96.0%）,whole group of 101 patients with a gravel success （success rate was 80.8%）,mean operative time was （40.2±3.4） min,postoperative back pain,fever,hematuria were 50.4%,13.6% and 49.6%.The success rate of lithotripsy,the average operative time,back pain, fever and other complications in experimental group were better than the control group,there was statistical significance （P〈0.05）. Conclusion Ureteroscopy and holmium laser treatment of ureteral calculi success rate,a success rate of lithotripsy treatment time are ideal,fewer complications,safer,and should be widely applied.
