摘要
目的探讨彩色多普勒超声在慢性肾功能不全患者肾脏结构及血流动力学检测中的应用价值。方法选择本院45例慢性肾功能不全患者为研究组,同期体检正常者45例为对照组,采用彩色多普勒超声诊断仪,观察肾脏形态,测量肾脏皮质厚度、实质厚度,并测量肾动脉收缩期最高流速(PSV)、舒张期最低流速(EDV)、平均血流速度(Vm)、阻力指数(RI)、搏动指数(PI)等。结果两组肾动脉血流动力学比较显示,研究组PSV、EDV、Vm明显低于对照组,RI、PI明显高于对照组,两组差异有统计学意义(P<0.05);肾功不全患者在肾功能失代偿期肾皮质厚度、肾实质厚度均明显低于对照组(P<0.05),在代偿期肾皮质、实质厚度与对照组差异无统计学意义(P>0.05)。结论彩色多普勒超声检测慢性肾功能不全,可有效显示血流动力学改变及肾脏形态结构改变的情况,对判定肾功能、临床治疗有重要指导意义。
Objective To investigate the value of color Doppler ultrasound in kidney structure and hemodynamics de- tection of patients with Chronic renal insufficiency. Methods 45 eases of patients with chronic renal insufficiency in our hospital were selected as study group,and 45 cases of normal people during the same period were selected as the control group,the color Doppler ultrasound were used to observe renal morphology,the renal cortical thickness,parenchy- mal thickness,and measure renal artery systolic velocity (PSV),the lowest diastolic velocity (EDV),mean blood flow ve- locity (Vm),resistance index (RI),pulsatility index (PI) were detected. Results The renal artery hemodynamics compari- son of two groups showed that,the PSV,EDV,Vm in study group were significantly lower than the control group,the RI,PI was significantly higher than that in control group,the difference was statistically significant (P〈0.05).In decompensatory stage,renal failure stage and uremia stage of renal insufficiency patients,renal cortical thickness and parenchymal thickness were significantly lower than the control group (P〈0.05),but in compensated stage,renal cortical thickness and parenchymal thickness were no significant different from control group (P〉0.05). Conclusion Color Doppler detection in chronic renal failure patients can effectively display hemodynamic changes and renal morphology changes,and has im- portant directive significance in renal function judge and clinical treatment.
