^(99m)Tc-DTPA肾小球滤过率测定在诊断糖尿病早期肾损害中的价值 被引量:5

摘要 目的评价99mTc-DTPA肾动态显像测定肾小球滤过率(GFR)在诊断早期糖尿病肾损害的价值。方法选取2009年6月至2013年5月在我院内分泌科就诊的2型糖尿病患者77例,其中男41例,女36例,平均年龄(52.34±10.21)岁。行99mTc-DTPA肾动态显像测定双肾及分肾GFR,同时测定24 h尿微量白蛋白(24 h UmAlb)、尿转铁蛋白(TRU)、尿微量白蛋白(MA)、血清肌酐(Scr)、尿素氮(BUN),并计算24 h UmAlb与Scr的比值(ACR)。分别以24 h UmAlb和分肾GFR差值为标准进行分组,应用独立样本t检验,分析上述指标的差异是否具有统计学意义。结果 24 h UmAlb升高组中,ACR、TRU、MA等均较正常组显著升高(P<0.05),GFR、血清光抑素C(Cys-C)、Scr、BUN无统计学差异(P值分别为0.411、0.300、0.794、0.908)。以分肾GFR差值为标准的分组中,差异组GFR总值、24 h UmAlb、ACR、TRU、MA等均较无差异组显著升高(P<0.05),Cys-C、Scr、BUN均无统计学差异(P值分别为0.766、0.104、0.909)。结论应用99mTc-DTPA肾动态显像测定GFR,能够了解分肾功能情况,对于糖尿病肾损害的早期诊断和随访具有较高价值。
