经皮椎体成形术与经皮椎体后突成形术治疗骨质疏松性椎体压缩骨折的疗效对比分析 被引量：19

摘要 [目的]评估经皮椎体成形术(PVP)和经皮椎体后突成形术(PKP)治疗骨质疏松性椎体压缩骨折的疗效。[方法]60例老年骨质疏松性胸腰椎压缩骨折患者随机分成2组,每组各30例。PVP组15例单椎体,10例2个椎体,4例3个椎体,共47个椎体接受治疗;PKP组18例单椎体,10例2个椎体,2例3个椎体,共44个椎体接受治疗,每次压力注射骨水泥后,采用停留几秒钟,等待针管内的压力完全释放后再行下一次压力注射方法。[结果]60例患者手术操作全部成功。PVP组16例完全缓解,14例部分缓解;PKP组20例完全缓解,10例部分缓解。术后PKP组椎体压缩率和Cobb角与术前比较有改善(P<0.05)。本组无骨水泥渗漏等合并症。[结论]PVP和PKP两种手术方法均具有显著止痛效果,PKP较PVP椎体高度恢复及脊柱后突畸形矫正效果更好。每次压力注射骨水泥后,停留几秒钟,等待针管内的压力完全释放后,再进行下一次注射的方法,有助于防止骨水泥渗漏进入椎管。
