摘要
目的 探讨子宫肌瘤患者采用不同的手术方式治疗后对生殖内分泌激素的影响.方法 回顾性分析2012年9月～2013年10月来我院治疗的125例子宫肌瘤患者的临床资料,按照治疗采用的手术方式将患者分为：A组共41例,采用子宫肌瘤挖除术治疗;B组共41例,采用次全子宫切除术治疗;C组共43例,采用全子宫切除术治疗.检测FSH、LH、E2、P;Vmax、Vmin、RI、PI,对结果进行统计学处理.结果 术后6个月的E2水平明显低于术前,差异有统计学意义（P＜0.05）;术后3个月的E2水平低于术前,但差异无统计学意义（P＞0.05）.A组患者的FSH、LH、E2、P术前、术后3个月、术后6个月差异均无统计学意义（P＞0.05）;B组、C组患者术后6个月的E2水平明显低于术前,差异有统计学意义（P＜0.05）.子宫切除术患者术后3个月及术后6个月的Vmax及Vmin明显低于术前,RI及PI值明显高于术前,差异均有统计学意义（P＜0.05）.结论 子宫肌瘤患者采用子宫肌瘤挖除术治疗对卵巢功能影响不明显,采用全子宫切除术及次全子宫切除术治疗均会对子宫肌瘤患者的卵巢血供及内分泌功能造成一定的影响.
Objective To investigate the influence of different surgical procedures on reproductive hormone of patients with uterine fibroids. Methods The clinical data of 125 cases with uterine fibroids that were selected from September 2012 to October 2013 in our hospital were retrospectively analyzed. All the patients were divided into three groups in accordance with the adopted surgical treatment： group A, included 41 cases, the patients used uterine fibroids excava- tion for treatment; group B included 41 cases, the patients used subtotal hysterectomy for treatment; group C included 43 cases, the patients used hysterectomy for treatment. FSH, LH, estradiol, progesterone; Vmax, Vmin, RI, PI were de- tected, and the results were analyzed with statistical methods. Results The estradiol after 6 months was significantly lower than preoperative, there was significant difference （P 〈 0.05）; the estradiol after three months was lower than pre- operative, but the difference was not significant （P 〉 0.05）. There were no significant difference in FSH, LH, estradiol, progesterone of group A among surgery, 3 months, 6 months after surgery （P 〉 0.05）; the estradiol levels of group B, group C after 6 months were significantly lower than before surgery, there was significant difference （P 〈 0.05）. Vmax and Vmin of hysterectomy patients after 3 months and 6 months were significantly lower than the preoperative, RI and PI were significantly higher than the preoperative, there was significant difference （P 〈 0.05）. Conclusion The patients with uterine fibroids treatment of uterine fibroids excavation significant effect on ovarian function, the use of hysterec- tomy and subtotal hysterectomy in patients with uterine fibroids treatment will be of ovarian blood supply and endocrine function have been affected.
