摘要
目的:探讨C3aR、C5aR在糖尿病肾病(DN)患者肾脏组织中的表达情况及其在DN发病中的作用。方法:选取DN患者的肾脏组织标本45例作为病例组,其中肾脏病理分级为Ⅰ+Ⅱ级13例、Ⅲ级21例、Ⅳ级11例;同时选取肾脏肿瘤患者手术切除的正常肾脏组织标本10例作为对照组。常规方法检测24 h尿蛋白定量、红细胞沉降率(ESR)、血肌酐(Scr)、血尿素氮(BUN)、C-反应蛋白(CRP)等指标。采用免疫组化方法检测C3aR、C5aR在各组肾脏组织中的表达。分析DN患者肾脏病理损伤级别与临床指标、肾组织中C3aR和C5aR阳性细胞百分比与临床指标的相关性。结果:C3aR和C5aR在各级DN患者肾脏组织中的表达均高于对照组,且随着DN肾脏病理损伤的加重,其表达逐渐增强(F=45.987,40.426,P均<0.001)。DN患者肾脏病理损伤级别与24 h尿蛋白定量、Scr、BUN、CRP等呈正相关(rS分别为0.529、0.595、0.516和0.425,P均<0.05);C3aR在肾脏组织中的阳性细胞百分比与24 h尿蛋白定量、Scr、BUN、CRP及ESR呈正相关(rS分别为0.342、0.613、0.489、0.315和0.321,P均<0.05)。C5aR在肾脏组织中的阳性细胞百分比也与以上指标呈正相关(rS分别为0.403、0.441、0.315、0.456和0.389,P均<0.05)。结论:C3aR、C5aR参与DN的发生与发展,在DN的病理损伤过程中起重要作用。
Aim:To detect the expressions of the receptors of C 3a and C5a(C3aR, C5aR) and investigate the poten-tial role of C3aR and C5aR in the pathogenesis of diabetic nephropathy (DN).Methods:A total of 45 cases of renal tissue from DN patients ( DN groups:13 cases at stageⅠ+Ⅱ,21 cases at stageⅢ, and 11 cases at stage Ⅳ) were selected ,and 10 cases of normal renal tissue from nephrectomy patients with kidney tumor were obtained as control group .The expres-sions of C3aR and C5aR in the above groups were detected respectively by immunohistochemistry .Some clinical test results such as:24 h urine protein,ESR,Scr,BUN,CRP etc.were measured by conventional methods .Results: The positive rates of C3aR and C5aR in DN groups were statistically significant higher than those in control group (F=45.987,40.426, respectively,all P〈0.001).In DN groups,positive correlations were found between some clinical test results (such as:24 h urine protein,Scr,BUN,CRP) and renal pathological stage (rS =0.529,0.595,0.516,0.425,respectively,all P 〈0.05);positive correlations were also found between clinical test results (such as:24 h urine protein,Scr,BUN,CRP, ESR)and the expression of C3aR in DN groups(rS =0.342,0.613,0.489,0.315,0.321,respectively,all P〈0.05),so&amp;nbsp;was the expression of C5aR in DN groups(rS =0.403,0.441,0.315,0.456,0.389,respectively,all P〈0.05).Conclu-sion:The high expressions of C3aR and C5aR may play an important role in the pathogenesis of DN .
