摘要
目的了解居家化疗传染性肺结核患者服药依从性状况,探讨其可能的影响因素。方法采用分层随机抽样方法,选取山东省6个地区共720例居家化疗肺结核患者作为研究对象,使用自行编制的问卷对其进行调查。结果 720例调查对象中,男性506例(70.3%),女性214例(19.7%);服药依从者316例(43.9%),服药不依从者404例(56.1%);未婚118例(16.4%),已婚551例(76.5%),离婚或丧偶50例(6.9%),以离婚或丧偶作为参照,在婚者服药依从性较好(OR=0.490,P=0.026);新农合621例(86.2%),城镇职工医保99例(13.8%),以新农合为参照,城镇职工医保患者服药依从性较差(OR=2.415,P=0.003);城镇户籍56例(7.8%),农村户籍664例(97.2%),以城镇户籍为参照,农村户籍患者服药依从性较差(OR=2.019,P=0.044)。结论山东6县区居家化疗传染性肺结核患者半数以上存在服药不依从行为,服药依从性与婚姻状态、医疗保障类型以及家庭户籍类型相关。
Objective To investigate the compliance of home chemotherapy and its influencing factors among infectious tuberculosis patients.Methods We used stratified random sampling to choose 720 infectious tuberculosis patients with home chemotherapy from 6 areas in Shandong province.The investigation was carried out with a self-administered questionnaire to collect information on compliance of home chemotherapy and its influencing factors.Results The patients were 506 men（70.3%）and 214 women（19.7%）.Among the patients only 43.9%complied with the home chemotherapy and 56.1%did not comply with the chemotherapy.Compared with the patients of divorced or windowed,the married patients had a better compliance of home chemotherapy （odds ratio [OR]=0.490,P=0.026）.Patients with urban worker＇s medical insurance had a higher incompliance rate compared with those with new rural cooperative medical system （OR=2.415,P=0.003）.The patients registered in rural areas had a higher incompliance compared with those registered in urban areas （OR=2.019 P=0.044）.Conclusion More than a half of the infectious tuberculosis patients in 6 urban and rural areas of Shandong province did not comply with home chemotherapy and marriage status,medical care and registered living areas related to the compliance of home chemotherapy.
