摘要
目的探讨内膜异位症（EMS）合并不孕症行体外受精（WF）刺激周期排卵治疗后影响妊娠结局的相关因素。方法回顾性分析2011年5月-2013年5月在本院接受IⅥr促排卵治疗的245例EMS合并不孕症患者的临床资料，根据患者妊娠结局分为妊娠组114例以及未妊娠组131例，对2组患者基本情况进行单因素及Logistic多因素回归分析。结果经单因素分析可知，不孕年限、EMs分期、IVF治疗前手术治疗、刺激周期治疗方案均与EMS合并不孕症患者妊娠率有密切关系（P〈0．05）。经Logistic多因素分析可知，不孕年限〉5年、EMSⅢ～Ⅳ期、IVF治疗前手术治疗、长方案治疗是影响妊娠率的独立危险因素。与IVF治疗前行手术治疗的患者相比，未行手术治疗患者IVF治疗后优质胚胎数、优质胚胎率显著下降，差异有统计学意义（P〈0．05）。与短方案治疗组相比，长方案治疗组受精数、卵裂数、优质胚胎数、优质胚胎率显著上升，差异有统计学意义（P〈0．0fi）。结论不孕年限〉5年、EMSⅢ～Ⅳ期、IVF治疗前手术治疗、长方案治疗是影响EMS合并不孕症行IVF促排卵治疗后妊娠率的危险因素。行IVF促排卵前对EMS合并不孕症患者行手术治疗及促排卵过程中应用长方案治疗可提高胚胎质量，从而提高妊娠率。
Objective To explore the influencing factors of pregnancy rate after the treat- ment of ovulation inducing through IVF in patients with endometriosis （EMS） and infertility. Methods The clinical material of EMS and infertility patients was retrospectively analyzed from May 2011 to May 2013 in our hospital. All the patients were divided into pregnancy group （ n = 114） and non- pregnant group （ n = 131）. The influencing factors of two groups were analyzed by univariate and multivariate Logistic regression analysis. Results The duration of infertility, EMS staging, surgery treatment before IVF were all associated with infertility pregnancy according to u- nivariate factor of regression analysis （P 〈 0.05）. According to the Logistic multivariate analysis, over 5 years＇ infertility, EMS III- IV stage, surgery treatment before IVF and long - term treat- ment were independent risk factors affecting pregnancy rates. The difference was statistically signif- icant since the high - quality embryos was significantly decreased （P 〈 0.05 ）. Compared with the short term treatment group, the number of cleavage, quality embryos, embryo quality was signifi- cantly increased in the long＇ term treatment group, the difference was statistically significant （P 〈 0.05）. Conclusion Infertility duration over 5 years, EMS III -- IV stage, surgery treatment before IVF, long term treatment in patients with EMS and infertility are risk factors affecting ovulation. EMS patients with infertility applying IVF can improve embryo quality and improve pregnancy rates.
