摘要
目的了解肿瘤患者鲍氏不动杆菌医院感染状况及耐药性特征,探索预防与控制鲍氏不动杆菌医院感染的监控措施,并为临床治疗提供依据。方法对2011年1-12月肿瘤患者发生医院感染分离出的鲍氏不动杆菌及其耐药性进行回顾性统计分析,采用美国临床实验室标准化委员会推荐的纸片扩散法进行测定初筛和确证试验,药敏试验为CLSI推荐的K-B法,采用WHONET最新版本进行数据统计分析。结果在检出的451株病原菌中检出鲍氏不动杆菌28株,检出率6.21%,其中多药/泛耐药鲍氏不动杆菌20株占71.43%;检出率居首位的为痰液标本占57.14%;临床分布主要在外科、ICU;鲍氏不动杆菌对常用抗菌药物均敏感,多药耐药鲍氏不动杆菌对除碳青霉烯类与多黏菌素E外的常用抗菌药物均耐药,泛耐药鲍氏不动杆菌仅对多黏菌素E敏感。结论肿瘤患者鲍氏不动杆菌,尤其是多药/泛耐药鲍氏不动杆菌医院感染情况日趋严重,应加强临床合理使用抗菌药物的管理,并对感染病例实施监控,防止多药/泛耐药鲍氏不动杆菌医院感染的暴发。
OBJECTIVE To understand the incidence of Acinetobacter baumannii infections in the tumor patients and analyze the characteristics of drug resistance and explore measures for prevention and control of A. baumannii infection so as to guide the clinical treatment. METHODS From Jan to Dec 2011, the A. baurnannii strains isolated from the tumor patients and the drug resistance were retrospectively analyzed, then the primary screening and the confirmatory trial were performed by means of the disk diffusion method recommended by the Clinical Laboratory Standard Institute, the drug susceptibility testing was conducted by using the K B method of CLSI, and the statistical analysis of data was performed with the use of latest version of WHONET. RESULTS Among the 451 strains of pathogens isolated, totally 28 strains of A. baumannii have been detected with the detection rate of 6.21%, among which there were 20 (71.43%) strains of multidrug-resistant or pandrug-resistant A. baumannii; 57.14% of the strains were isolated from the sputum specimens; the strains mainly distributed in the department of surgery and ICU. A. baumannii strains were susceptible to the commonly used antibiotics; the multidrug-resistant A. baumannii strains were resistant to all the commonly used antibiotics except for carbapenems and polymyxin E; the pandrug-resistant A. baumannii strains were only susceptible to polymyxin E. CONCLUSION The incidence of nosocomial infections caused by the A. baumannii strains, especially the multidrug-resistant or pandrug-resistant A. baumannii strains , has become increasingly serious; it is necessary for the hospital to strengthen the reasonable clinical use of antibiotics, monitor the cases of infections so as to control the nosocomial infections caused by the multidrug-resistant or pandrug-resistant A. baumannii strains.
