摘要
目的了解住院患者下呼吸道感染病原菌分布及耐药性,为临床合理用药提供依据。方法对2009-2011年住院患者下呼吸道感染痰标本所分离的病原菌,使用法国生物梅里埃公司ATB微生物分析仪和细菌鉴定试剂盒进行菌株鉴定,采用K-B法进行药敏试验及结果判定,采用WHONET5.4软件进行数据处理和分析。结果 3年医院下呼吸道感染的病例中共分离出2 655株病原菌,其中革兰阴性杆菌1 803株占67.9%,革兰阳性球菌547株占20.6%,真菌305株占11.5%;感染率最高的病原菌前5位依次为铜绿假单胞菌597株占22.5%、肺炎克雷伯菌542占20.4%、大肠埃希菌345株占13.0%、金黄色葡萄球菌278株占10.5%、白色假丝酵母菌207株占7.8%;常见的病原菌对常用的抗菌药物存在不同程度的耐药。结论下呼吸道感染病原菌以革兰阴性杆菌为主,应加强对下呼吸道感染病原菌的监测与控制,严防耐药菌株在医院内播散流行。
OBJECTIVE To investigate the distribution of bacteria and antibiotic resistance of pathogens isolated from inpatients with lower respiratory tract infection, so as to provide the reference for the rational antimicrobial therapy in clinic. METHODS Bacteria isolated from sputum in the of nosocomial lower respiratory tract infection inpatients in our hospital from 2009 to 2011 were collected. ATB Microbial analyzer and Bacteria identification kit were used for strain identification. K-B test was used for drug suscptiba test and result judgement. WHONET5.4 software was used for data analysis. RESULTS Totally 2 655 strains were isolated from the nosocomial lower respiratory tract infection inpatients in our hospital during three years, among which 1 803 strains of Gram-negative bacilli were isolated, accounting for 67.9%, 547 strains of gram-positive bacilli, accounting for 20. 6%, 305 strains of fungi, accounting for 11. 5 % ; The top five pathogens were: Pseudomonas aeroginosa (597 strains, 22. 5 %), Klebsiella pneumoniae (542 strains, 20.4%), Escherichia coli (345 strains, 13. 0%), Staphylococcus aureus (278 strains, 10. 5 %) ,and Candida albicans (207 strains, 7. 8 %) ; These pathogens were resistant to the common antibiotics with the different resinam degrees. CONCLUSION Gram-negative bacilli are the major pathogens causing lower respiratory tract infection, Monitoring and controlling batena resstance should be strengthened to prevent spreading and epidemingy of resistant strains in hospital.
