摘要
目的研究护理干预对外科手术患者焦虑情绪的影响。方法研究2011年6月—2012年6月深圳市人民医院龙华分院收治的226例外科手术患者,随机分为对照组及观察组2组,每组113例。对照组患者给予常规围术期护理,观察组患者在常规护理基础上给予全方位围术期护理干预。观察并比较2组患者护理前后焦虑自评量表(SAS)、抑郁自评量表(SDS)、汉密尔顿焦虑量表(HAMA)评分、睡眠、情绪状态、疼痛程度以及临床满意度情况。结果护理后2组患者的SAS、SDS和HAMA评分均有明显的降低(P<0.05);观察组患者的SAS为(31.2±10.8)分,SDS为(30.3±11.2)分,HAMA评分为(7.7±3.2)分,明显低于对照组(P<0.05)。观察组患者术前、术后的睡眠、情绪状态评分均显著高于对照组,术后疼痛程度显著低于对照组(3.0±0.4 vs.3.7±0.2,P<0.05),观察组患者临床总满意率93.8%,显著高于对照组(73.5%,P<0.05)。结论全方位护理干预可能有效缓解患者焦虑和疼痛,改善睡眠,值得临床推广。
Objective To investigate the influence of nursing intervention on anxiety in surgical patients. Methods 226 patients were randomly divided into control and observation group（ 113 cases in each group）. The control group received routine care, the observation group received comprehensive nursing intervention based on control group. Self-Rating Anxiety Scale （SAS）, Self-rating Depression Scale （SDS）, Hamilton Anxiety Scale （HAMA） score, sleep state, emotion state, degree of pain and clinical satisfaction were compared before and after the nursing intervention. Results SAS, SDS and HAMA score in both groups were significantly decreased（P 〈 0.05 ） ,while the SAS, SDS and HAMA score in observation group were significantly lower than those of the control group（31.2 ± 10.8 vs. 44.1 ± 13.1,30.3 ± 11.2 vs. 40.1 ± 9. 3,7 ± 3.2 vs. 14.5 ± 6.8, respectively, P 〈 0.05 ）. The preoperative and postoperative sleep and emotional state scores in observation group were significantly higher than those of control group while the postoperative pain was significantly lower （ 3.0 ± 0.4 vs. 3.7 ± 0. 2 ,P 〈 0.05 ）. The satisfaction rate in observation group was significantly higher than that of control group （93.8 % vs 73.5 %, P 〈 0.05 ）. Conclusion Comprehensive nursing intervention might effectively relieve anxiety and pain, improve sleep and be worthy of clinical use.
