摘要
目的探讨双重造影在胃良恶性肿瘤鉴别诊断中的临床应用价值。方法选取收治经临床病理确诊的胃良恶性肿瘤80例患者,采用盲法分为观察组及对照组各40例,对照组患者进行单一造影检查(胃充盈超声造影);观察组在对照组基础上联合静脉超声造影进行双重造影检查。将两组患者所得到的影像诊断与病理诊断进行对比分析研究,观察患者在采用不同造影后对疾病的影像诊断效果。结果在胃良恶性肿瘤鉴别准确率方面,胃单一充盈造影诊断胃恶性病变的诊断符合率为73%,胃双重造影诊断胃恶性病变符合率为93%,采用双重造影的诊断率明显高于胃单一充盈的准确率,差异具有统计学意义(P<0.05)。结论使用双重造影来鉴别诊断胃良恶性肿瘤,可清晰的显示胃壁不同层次间的血流灌注差别,恶性肿瘤的微血管灌注显著增加,对于病变大小和侵润范围显示更为清晰,准确率较高,因此该方法值得在临床上推广和使用。
Objective To study and probe into the double contrast in differential diagnosis of benign and malignant gastric tumor in clinical application. Methods In our hospital in 2010 March to ~2012 December, 80 patients with clini- cal pathological diagnosis of benign and malignant gastric tumor were divided into the observation group and control group. In the observation group, on the basis of the control group with intravenous contrast ultrasound double contrast examination~ in the control group, single angiography on patients （gastric ultrasonography angiography）. The image di- agnosis and pathological diagnosis study on the comparative analysis, image diagnosis effect observation of disease in dif- ferent angiography. Results The accurate rate in the differential diagnosis of benign and malignant gastric tumor, con- sistent with the diagnosis of gastric single filling angiography in the diagnosis of gastric cancer was 73%, consistent with the gastric double radiography diagnosis of gastric malignancy rate was 93 ~, the diagnostic accuracy rate by using the double contrast was significantly higher than that of single filling the stomach, there is essential difference between nature between the two, with statistical significance （P〈0. 05）. Conclusion The use of double contrast to the differential diag- nosis of benign and malignant gastric tumor, can display the perfusion difference between the different layers of the gas- tric wall clear, microvascular perfusion malignant tumors inereased significantly, the lesion size and infiltration scope dis- play more clearly. High accuracy, so the method is worthy to be popularized and used in clinic.
