摘要
目的探讨PPH手术在治疗混合痔合并直肠内脱垂中单、双荷包缝合的效果及选择原则。方法 168例患者随机分成两组,PPH手术采用单荷包缝合治疗84例,其余84例采用双荷包缝合。结果两组在术中耐受程度、术后恢复时间方面基本相似,但手术时间、术中下腹部牵拉痛、假性憩室形成、吻合口裂开等方面,单荷包缝合优于双荷包缝合组。肛管皮肤复位、直肠内脱垂消失或两者效果均佳方面,双荷包优于单荷包缝合组。结论混合痔合并直肠内脱垂在选择用PPH手术治疗时应采用个体化治疗措施,针对混合痔重而合并直肠内脱垂轻者,或混合痔相对轻而直肠内脱垂重者,最好选用单荷包缝合,两者均重者选用双荷包缝合,并且两荷包线间距不应>2cm。
Objective To discuss the effective choice of PPH performing single or double purse-string in treatment of mixed hemorrhoid complicated with internal prolapse of rectum. Methods 168 patients were divided randomly into two groups. 84 patients were treated with PPH performing single purse-string and the other 84 patients were treated with PPH performing double purse-string. Results There was no significant difference between the two groups in torance de- gree of operation and postoperative recovery, but single purse-string group was better than double purse-string group in operative duration, underbelly stretch pain, false diverticulum formation and anastomotic dehiscence, double purse-string group was better than single purse-string group in anal canal skin reduction, internal prolapse of rectum disappearance or both of them. Conclusion Individual treatment should be adopted in the treatment of mixed hemorrhoid complicated with internal prolapse of rectum, when patients mixed hemorrhoid symptoms were severe and internal prolapse of rectum symptoms were mild or mixed hemorrhoid symptoms were mild and internal prolapse of rectum symptoms were severe. It is better to use PPH performing single purse-string, if patients mixed hemorrhoid symptoms and internal prolapse of rec- tum symptoms were both severe.
