外伤患者大量输血后血钾的变化及其影响因素探析 被引量:4

摘要 目的探讨外伤患者大量输血后血钾的变化情况以及相应的影响因素。方法抽取2011年1月~2013年7月本院收治的外伤患者98例,将其按照是否接受大量输血分为输血组与非输血组,对这两组患者术前术后血钾、血糖以及血pH情况进行监测,并对比分析。结果术前两组患者血钾水平差异无统计学意义(/9〉0.05),血糖与血pH值差异有统计学意义(P〈0.05);术后两组患者血钾水平差异无统计学意义(P〉0.05),血糖与血pH值差异有统计学意义(P〈0.05),术后12h内输血组患者高钾血症发生率较非输血组显著升高(P〈0.05)。结论外伤患者出现高钾血症的主要原因为受血糖与血pH值的影响,与大量输血治疗无显著相关性,值得关注。 Objective To analyze and investigate the serum potassium changes after massive blood transfusion and its influencing factors on trauma patients.Methods 98 cases of trauma patients,who were treated in our hospital from Jan- uary 2011 to July 2013 were divided into blood transfusion group and non-blood transfusion group based on whether to accept massive blood transfusion.The serum potassium,blood glucose and blood pH value of patients in two groups be- fore and after operation were monitored and analyzed comparatively.Results Serum potassium level on patients in two groups before operation had no significant difference (P〉0.05),but there was significant difference on blood glucose and blood pH value (P〈0.05).There was no difference on serum potassium level after operation between two groups (P〉0.05), but there was significant difference on blood glucose and blood pH value (P〈0.05).Within 12 hours after operation,the incidence of hyperpotassemia on patients in blood transfusion group was significant higher than that in non-blood transfusion group (P〈0.05).Conclusion The main cause of hyperpotassemia occurred in trauma patients is the influence of blood glucose and blood pH value,and it has no significant correlation with massive blood transfusion treatment.It is worthy of attention.
