摘要
目的探讨2型糖尿病患者血清超敏C反应蛋白(hs-CRP)、白介素-6(IL.6)、肿瘤坏死因子-α(TNF-α)在糖尿病发生、发展过程中的浓度变化及与疾病并发肾病、糖化血红蛋白(HbAlc)程度、病程间相互关系。方法用双抗体夹心酶联免疫法和特种蛋白仪分析100例2型糖尿病患者及50例年龄和性别相匹配的正常对照者血浆CRP、IL-6、TNF-α和尿微量清蛋白(U-mALB)的水平。结果(1)2型糖尿病患者血中hs-CRP、IL.6和TNF-a的水平明显高于对照组(P〈O.01);且糖尿痛肾病患者表现更明显。(2)2型糖尿病患者随着HbAlc的增高,血清中hs-CRP、IL.6和TNF-α也明显增高(P〈O.01)。(3)2型糖尿病患者血清中hs-CRP、IL-6和TNF-α随着病程的增加而表现增高趋势(P〈O.05)。(4)将糖尿病组血清中hs-CRP、IL-6和TNF-α与糖尿病病程、HbAlc及U—mALB进行直线回归相关分析,表现出低度的相关性(P〈0.0S)。结论炎性因子hs-CRP、IL-6及TNF-α在糖尿病的发生、发展过程中相互作用,观察其浓度变化对探讨糖尿病的发病机理、预防及指导用药均有重要价值。
Objective To investigate the role played by serum relative inflammatory cytokine (hs-CRP, IL-6, TNF-α)in the path- ogenesis of diabetes mellitus. Methods Serum hs-CRP,IL-6,TNF-α and U-mALB leves were measured with ELISA and biochemis- try analysis system for special proteins in 33 cases diabetic patients without complications,67 cases patients with early nephro or retinopathy and 50 eases controls. Results ①erum leves of hs-CRP, IL-6 and TNF-α were significantly higher in the diabetic pa- tients than those in controls (P〈0.01). ②The serum leves of these relative inflammatory cytokine factors were mutually positively correlated with HbAlc(P〈0.01). ③The serum leves of these relative inflammatory cytokine factors were positively correlated with disease course(P〈0.05). Conclusion The relative inflammatory cytokine factors hs-CRP,IL-6 and TNF-α participated in the development and pathophysiology of diabetes mellitus and monitoring their plasma level changes would be clinically useful.
