功能性二尖瓣反流程度与慢性心力衰竭预后相关性研究 被引量:3

摘要 目的研究功能性二尖瓣反流程度与心力衰竭患者的预后的相关性。方法选取2008-2011年住院的心力衰竭患者(NYHA II^IV级)113例,以二尖瓣反流量分组,分为轻(58例)、中(35例)、重(20例)三组。用超声心动图测量左心室射血分数(LVEF)、左心室舒张末内径(LVEDD)、左心室收缩末内径(LVESD)、肺动脉压(通过三尖瓣反流程度计算)、E/A。ELLSA测血浆BNP,心电图测QRS宽度。统计1年内心力衰竭再住院数及死亡数。如上数据进行统计分析。结果 LVEF、LVEDD、LVESD、肺动脉压、BNP、QRS宽度与二尖瓣反流程度呈正相关,重度二尖瓣反流组再住院率及死亡率(再住院率60%,死亡率20%)均高于轻度反流组(再住院率22.4%,死亡率5.17%)及中度反流组(再住院率28.6%,死亡率11.4%),差异有显著性(P<0.05)。结论慢性心力衰竭患者常见功能性二尖瓣反流,其反流的严重程度与预后相关。
